ABA Interview Form 1 Indirect Interview Form Interviewer: Behavior Specialist Date: June 5, 2022 Respondent: Jayme Lopez Relationship to client: Mother Address: 1445 S. Timberland Way, Detroit, Michigan Telephone: Home: ______________ Work: ______________ Cell : 313-711-2203 E-mail: lopez222@reachingout.com Name of client: Hector Lopez Date of Birth: 10-1-2018 Gender: Male Age: 5-years Strengths of Client: His attending skills are good for things of interest to him. He knows his alphabet and can count to 100. He has a very large vocabulary and often sounds like an adult when he talks about his interests. He can build elaborate cities out of Legos. Weaknesses/Deficits: He does not enjoy an activity that takes him away from his favorite activities and toys. He often runs away from us in public and it makes us so nervous. He won’t listen if we yell stop which makes it unsafe to take him in public right now. Also, he is possessive over his toys specifically his trucks. He will engage in tantrum behavior and yell and even try and push sometimes. Behaviors of Concern: Hector will run away from us when we are at the park or in public. He also holds onto his favorite trucks and doesn’t share. He will yell, scream and even try and hit or push if they try and take his toys away. We are worried the school will move his classroom placement. What has been your usual response? I yell “stop” when he runs away. He laughs and continues. I also usually just let him have the trucks and don’t fight it. What typically occurs just prior to the behavior of concern? The behavior seems to occur primarily when we are out in the community. Also, his possession of toys happens around trucks and sometimes trains. It happens at school and at home. The behavior happens when someone tries and grabs the toys or says “give it to me.” Have you noticed whether the behavior occurs more often in specific settings? Yes X No__ If yes, list settings: Typically, the elopement behavior occurs when we go to park, store or parking lot. Also, the tantrum behavior occurs when he is told to give up a favorite toy. Does the behavior occur more in the presence of specific individuals? Yes _X__ No In whose presence does the behavior occur most frequently? Sometimes, the behavior happens more with me (mom) than his other caregivers. Are there certain times of day in which the behavior occurs more frequently? Yes ___ No X 1
ABA Interview Form 2 If so, what times of day does the behavior occur most frequently? N/A Antecedent variables: Does the behavior occur after the following? Please check all that apply. No Attention Demand Made Denied Access to Something Desired Alone X(elopement) X (for tantrum) How frequently does the behavior occur each day/week? Four times per week; but it happens whenever we are in a public place such as park. Tantrum happens daily a few times a day often. On average, how long does each occurrence of the behavior last? The running away happens quickliy and after a few minutes he will either come back or someone wil chase him down. The tantrums are short lived as long as he gets the item back. If we take the toy away, the tantrum will increase and he will yell, hit, or scream more sometimes for thirty minutes. What are his/her interests/activities? He loves talking with me and his babysitter and he really enjoys working in his alphabet and numbers books. He is crazy about Legos! And, he likes all books about dinosaurs. His most preferred items are trucks and trains. What are his/her favorite snacks/foods? His favorite snacks are dried pineapple; hamburgers; and peanut butter crackers. What are his/her favorite toys? He loves trucks, trains and he likes to play Thomas the train games on the tablet. Would you be able to provide some reinforcers for use during therapy and for use in the intervention program? Yes ABA therapy is most effective when the intervention can be implemented every day in all of the client’s environments. Would you be willing to be trained in the implementation of the Behavior Intervention Plan (BIP)? Yes X No ____ Would his dad? Yes X No ____ Thank you so much for your time. Our next step will be to schedule direct observations. Can we set up some times that are convenient for you? Yes. You are more likely to see he behavior when we go out. Would you want to do that?
