Report
Peak 1680 was approaching the runway 4 right. The wind direction was northwest, 330 degrees at 21 KTS. 350 degrees at 30 KTS gusting to 45 KTS. As the investigator, it would be important that the wind velocity was increasing in velocity, and it seemed as if the wind was slightly changing its direction. The wind was 330 degrees on descend at 25 KTS and 320 degrees at 23 KTS. The weather leading up to the accident was reported to be heavy rainfall and dark just like what “JD” described in the interview. It was also mentioned during descend that the visibility was less than a mile the night Peak 1680 crash happened. o The runway was wet, and the brakes were applied a little too late. o The flight profile was not or did not seem normal. o “JD” claimed that during the landing, the aircraft’s centerline was on the left of the centerline. The scene tells me besides the fact the plane obtained massive damage from the accident is that the scene is preserved. The first photo that was taken was the main landing gear of the plane and it was measured 189 degrees from the main fuselage. It seemed that the second measurement that was done was the distance of the threshold marking of the runway 4 right. The distance was not shown, but it is approximately 223 degrees. Begin gathering information and the facts that are correlated with the accident so that it can be analyzed. The evidence gathering must be reported or recorded to make sure that all the evident are in fact not tampered. Like what I have mentioned above, the crash scene was preserved. It tells me by watching the simulation is that measuring the components in different angles and taking a closer picture is necessary for later review. Recorded interview of the witnesses is a must
to identify the actions that were taken or the activities that were happening prior to the accident. After interviewing the witnesses, re-watch the videos and check for the inconsistencies in the stories to determine if a later interview is necessary for the unclear answers from the previous interviews. Sequence of Events/Contributing causes. In the past module, creating a flow chart is one of the methods to be able to analyze the accident.
