Identify a condition for a patient and determine the clinical pathway
that the patient would take through at least three types of health services (for example, primary care, acute care, and rehabilitative care). Provide an overview of the condition and clinical pathway, including a diagram of the pathway.
I chose to use the condition of someone being diagnosed with diabetes because I recently had to go through this with my brother. So the clinical pathway we followed is listed below.
1.
Primary Care: We initially went and made a visit to his primary care physician to assess his symptoms and perform tests to confirm his diagnosis. The PCP then came up with a treatment plan as well as prescribing him with medication to help keep his glucose levels in balance.

Endocrinologist: We then went to an endocrinologist to get knowledge in managing his diabetes and coming up with a dosage amount for his insulin pump as well as implanting an omni-pod to help my brother keep track of his blood sugar as well as the device providing immediate reports in any fluctuations to the endocrinologist so that way, he can monitor the readings in real time and keep track of his progress.

Education and support: My brother then started going to a hospital weekly in another city as they had set up a diabetes info course in providing information and education to those who have diabetes as well as their family members so they can get more familiarized with it and can make changes to their day-to-day lifestyle to support him. This made my brother feel less alone and get the support and help that he needed as well as talking to other diabetics and how they have been able to manage over the years.

Dietician: We met up with a dietician to come up with better and healthier eating habits to support him in having a lower glycemic index and to prevent his levels from fluctuating so much.

Once the clinical pathway has been determined, evaluate the process, looking for bottlenecks and places where the patient will feel neglected or treated as a subject rather than as a person. Provide suggestions as to how the healthcare delivery system could
be improved for their patient.

Lack of patient centered care: I think there were moments where my brother felt a bit neglected because some providers weren’t prioritizing his individual needs and preferences. Suggestions to fixing this issue would be if healthcare providers were more involved in patient decision making and listening to their concerns, learning their lifestyle habits and providing empathy so that the patient feels heard and feels like it’s more of a safe space to communicate more openly.

Fragmented Care: We experienced a lot of records and progress notes not being sent over from provider to provider which resulted in a bit of a poor treatment plan as
the healthcare providers weren’t communicating effectively in their findings. This cause issues a few times in terms of his medication dosage. To improve this factor EHRs would definitely allow seamless correspondence between providers that way they can have all the information they need from other providers when patients come into their appointment. This also ensures that all providers are hands on in terms of care for the patient.

Limited Access in education: We did have to research for a while to find a place that provided education and support groups for those who have diabetes. None of the local facilities in where he lives provided any educational resources for them so he had to start travelling to another city to obtain the education he was looking for. To improve this factor I think it would be nice and accessible if they offered a teleconference of educational meeting for those to tune in no matter where they reside in order to get access to the help that they need.
