In paragraph form, construct a discussion that identifies those priority SDHs using data
from the objective and subjective data sets where appropriate to support your argument.
Include in your discussion some strategies you might utilize to address challenges with or
help strengths identified in those three priority SDHs.
Social determinants of health Refer to any conditions that involve areas where people live, learn, work, and play in age that affect any health condition and outcome. The five critical areas of social determinants of health (SDOH) include economic stability, education, health and health care, the neighborhood and the built environment, and social and community context (US Department of Health and Human services [HHS], 2020). Society can be unequal in many areas, including how low-income families in underserved and under-resourced areas live. Some affected areas include living in a food desert, housing issues, and air and water quality may also contribute. Downstream economic topics such as employment stability, income, debt, medical bills, food security, and housing stability all play a part. Childhood educational development, vocational training, higher education, language, and literacy, including health literacy can create additional burdens for families affected by SDOH. Even community engagement, support systems throughout this community, discrimination, and the neighborhood and built environments also contribute to issues with transportation, housing quality, crime, safety, and walkability, which can affect how people exercise or have access to exercise (Florida Health, 2021).
For the patient in question, the first SDOH appears to be in the form of economic stability. This young woman is working three jobs to support her and two children. This stress has led to. Chronic migraine headaches and elevated blood pressure. Assisting her with referrals to community programs that can help stabilize her finances, such as outreach programs, housing programs, and utility payment programs, can assist with easing her burdens. This can include

additional job training and increased security in other areas, such as food and shelter access (HHS, 2020).
Additionally, the social determinant of healthcare access is seen in the review. The patient has seen the burdens associated with this in the form of weight gain and lack of a Primary Health care provider that can assist her with her needs more regularly. Other health factors could be seen in her family history. It appears to be a history of obesity, diabetes, and hypertension, leading to chronic illnesses in a generational pattern that has not been previously addressed. This can directly relate to diet, lifestyle choices, and learned behaviors. Education to the patient related to smoking cessation, gaining access to fresh fruits and vegetables to better her and her children's diets, and promoting exercise with tips on where to go to walk, cycle, swim, or work out in the community (HHS, 2020). And lastly. The patient has been affected by social and community contexts and the form of community support she may have afforded. She works three jobs and cares for two children as a single mother. Referrals to community-based programs to keep her children active and provide her with family options for support (HHS, 2020).
