Hello professor and class, Client challenges will always arise. Contributing factors can be socioeconomic, level of education, biases, and racially charged preconceived notions and ideas. The United States is a melting pot of culture, and healthcare is no different. During the start of the Covid-19 pandemic, information was scarce and scattered. It seemed like the world stood still. Having a diverse workforce, the office of the NP is flooded with patients seeking answers and solutions to the pandemic. The office has Asian descent staff members. Their roles range from font desk to medical assistant and medical doctors. On this particular day the NP noticed that a patient seemed distracted. This made the interview process challenging as many questions had to be asked more than once due to lack of focus by the patient. As the NP finally gets their full attention, the patient asks “how could you work among these Chinese folks knowing the virus came from Wuhan? They better not come near me!” The comment is one that unfortunately was heard repeatedly throughout this pandemic period. In healthcare 19% of physicians and 8.4% of nurses are Asian(Shang,2021). The client also commented that the President himself called it the “China Virus”(Shang,2021) and that they “shouldn’t be allowed around people.” The patient is displaying racist and discriminatory behavior in the forms of verbal abuse. This puts a strain on the client-provider relationship because the safety of the staff is a major concern. On March 16, 2021 8 people were killed in what is known as the Atlanta Spa Shootings (Bogel-Burroughs et al.,2021). Of the 8 victims 6 were Asian. Situations like these add unnecessary stress and can lead to heated arguments and physical abuse. The NP needs to face the issue head on and try to use
communication that is “clear, factual, and specific”(Torjesen,2023). The NP needs to start with a statement the patient can’t disagree with like “I can see you are in discomfort, and I would like to help you get home to your family.” Avoid using the words “racist” or “offensive” but instead repeat their words back to them (Torjens,2023). In this instance the NP can say “you are saying that the Asian staff is going to infect or hurt you based on the origin of the virus and something the President said.” The NP should express their feelings towards the incident by saying “I am upset to hear that you feel that the staff is going to hurt you especially when they are US citizens that have nothing to do with what anyone said about the virus. They deserve respectful treatment, especially in a time where much is uncertain about the virus.” Explain that there will be consequences for their behavior in a direct manner by saying “If you insist on not having my Asian staff assist you, you may need to go elsewhere for any follow up care.” Set out expectations and use their name to be direct rather than sound generalized (Torjens,2021). “Mr Smith in order to get the care you need today, you will need to be respectful to my staff. This will ensure you get the best quality care and we will get you home in a timely manner and avoid more exposure than necessary.” The incident must be reported as soon as the visit is over. This will help have documented incidents that can help guide future encounters with this patient and others like him. An incident report is a good starting point and making sure the NP gets support for themselves if needed.
