Applying Research Skills Healthcare systems often struggle with staffing shortages, leading to increased workloads for existing staff and potential impacts on patient care. Less staff leads to more employee burnout, leaving room for errors that could potentially be fatal for the patient. Patients do not receive the proper one-to-one care they deserve because the staff take on workloads that consume their every minute. As a registered nurse, who currently works in the Intensive Care Unit at a hospital regulated by the federal government, we have rules in place that a nurse is not allowed to take on more than 2 critical patients. However, if a patient is a medical patient, or what is called a “step down” patient , my patient-to-nurse ratio can increase from three to one as high as four to one depending on the level of care those patients require. When staffing falls short other nurses from different hospital areas will float to my unit. Many of whom have never taken care of critically ill patients. This leads to a lack of knowledge regarding certain medical interventions such as ventilators, medication drips utilized for different effects on the human body, and what to do when a rapid response is called on a patient who is a full code. I transitioned into the ICU from a Long-Term Care facility. I was responsible at times for up to 40 residents. In this case, medications would not be administered in a timely manner, wound treatments would not be done until late in the shift, and I could not give residents the proper nursing care they needed because I was alone at times, I should have only needed to be responsible for half the number of patients. Identifying Academic Peer-Reviewed Journal Articles I utilized the search engine Summon, which is available in the Capella Library database. I refined my search using a peer-reviewed option to obtain peer-reviewed journals. I searched
using words such as “staffing shortage,” “healthcare,” nurse-to-patient ratio ,” and nurse recruitment .” This brought me to databases such as ProQuest Central, PubMed Central, and Ovid. I chose articles published within the last 3 years. Assessing Credibility and Relevance of Information Sources To certify the credibility of the peer-reviewed journals I chose, I used a three-year span for publication. I also reviewed the accreditation of the authors of each journal and how that correlates with their knowledge of healthcare, whether it be their education level or professional expertise in the field. I read each article I chose to validate that it relates to the topic chosen at hand. The articles relate to the facts and opinions about the staffing shortage and the effect it has on both employees and patients. Annotated Bibliography Vuong, L. (2020). Staffing Ratios and Burnout. AJN, American Journal of Nursing, 120 (5), 13- 13. https://doi.org/10.1097/01.NAJ.0000662724.83879.81 This article demonstrates the viewpoint of a Registered Nurse who has worked in the ICU in 2 different states. One state had nurse-to-patient ratio law and the other state did not. The author cites a report from the National Academy of Medicine (NAM) regarding three substantial burnout symptoms and how it affects nurses and the care they provide to their patients. The author also states that having worked in both environments, facilities with mandated staffing ratios lessen burnout by reducing nurses' workloads. This renders better care because it allows nurses more one-on-one time with patients and their families.
