Christ and Diwan (n.d) identified seven critical domains that social workers should consider when assessing older clients: Physical well-being and health, Psychological well-being and health, Cognitive capacity, Ability to perform basic Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), Social Functioning, Physical Environment, and Assessment of Family Caregivers. Neglecting any of these domains may result in the omission of crucial information necessary for the intervention phase. Magda appears to be in relatively good physical and mental health, as noted by Walden University (2022). However, her independence was compromised six months ago when she suffered a hip fracture, and she has recently been diagnosed with dementia. There are no reports of other health issues, but it is evident that she may require substantial assistance in her home. Helen, her caregiver, reported taking on responsibilities such as purchasing Magda's groceries, maintaining her home, managing her finances, and overseeing her medications (Walden, 2022). To better understand Magda's capabilities, it is essential to engage her in a discussion about her ability to perform activities such as cooking, bathing, and traveling independently. Magda also benefits from a strong support system provided by her family and her church (Walden, 2022). Her family appears to be closely-knit and willing to assist each other when needed. However, there are evident gaps in Magda's assessment, specifically regarding her cognitive capacity and the assessment of her physical environment. Given Magda's previous independence, it is crucial to involve her in every step of her treatment process. Her consent and preferences should have been considered before allowing her grandson to move in with her, as it is her home, and she might have had reservations. It is also important to assess her mental health to understand how her declining independence has affected her. The strengths perspective can be utilized to identify areas where she needs assistance and where she still feels capable of managing independently. Furthermore, Magda's professional support team should be consulted to gather her health history and assess areas of concern. Inquiring about her previous illnesses and potential risks can guide interventions aimed at maintaining her health and reducing risks. Additionally, evaluating the impact of her dementia and recent fall on her ability to live alone will help determine if she requires assistance from a caregiver in her home.
