PUNS List/HCBS Medicaid Waiver
What is PUNS?
PUNS or Prioritization of Urgency for Needs of Services is waitlist for individuals
who are trying to get home support-based services through a Home and
Community Based Service (HCBS) waiver.
Why sign up for PUNS?
First, it takes about 5 years or 60 months to get selected from the PUNS list for
(HCBS) waiver for support-based services, so it is best to sign up as soon as
possible. Especially if your child is under 18 you may want to sign them up while
they are in school age years so that they have funding when they turn 18. Once
your child is selected for services, they are guaranteed for life to have their
support-based services.
Who is eligible for PUNS?
Anyone who has a diagnosed Intellectual disability or Developmental disability is
eligible to be on the PUNS list for funding. Examples of Intellectual and
Developmental disabilities covered by the PUNS list include
Autism, Down
Syndrome, behavior disorders, brain injury, Cerebral Palsy, Fetal Alcohol
Syndrome and Spina Bifida
. Please contact your local (ISC) provider to see if
your child’s disability qualifies for placement on the PUNS eligibility list.
What do I get if I am selected from the PUNS list?
The funding from the (HCBS) waiver can be used in a variety of different ways to
help the client you are holding guardianship of. As of January 1
st
2023, the monthly
funding amount for an adult receiving home-based support services is $2,742 per
month to be used on home-based support services. Effective January 1
st
2024 that
amount will increase to $2,829 a month to be used for home base support. This
funding can be used in many ways including transportation, food, clothing, day
programming, therapy etc.
How to Sign up for Placement on the PUNS list?
First you need to find a local Independent Service Coordination (ISC) agency. I
will list the COOK county location below. When you make your first call to the
(ISC) agency please have the following on hand:
Child social security number,
Medicaid number (if they have Medicaid), Birthdate, Disability, Basic Contact
Information.
When you call the first time, please ask for a PUNS intake and this
call should only take 5 minutes since they are gathering basic information to put
your child on the waiting list. After this call you will be contacted by (ISC)
representative for an in-person meeting. For this meeting you will need to bring:
Most recent IEP and re-evaluation paperwork and Recent documentation
from doctors
. This meeting will likely take about 60-75 minutes and will ask a
variety of questions including how you would like to use the funding for your
child.
