KNES 4610 Project 1 – ASK Pre-Meeting/First Meeting (Subjective Assessment) and Pre-participation Health Screening (15% of Final Mark) The object of this assignment is to find two people who ideally are not already engaged in a regular exercise program, and who are willing to be interviewed and screened as though they were coming to you for exercise testing and training. Note you do NOT have to perform any physical assessments. You should create a brief consent form (adapt the CSEP one if you would like) for your client that says you will be gathering information about their medical history and activity background and will be sharing it with your teacher. You do not need to use full names, or you may use a pseudonym if your “client” prefers. Any information you obtain that your “client” does not wish to share you can leave out. I will not stipulate who your client can or cannot be but remember the goal is to give you some experience with screening individuals and you will likely get more out of the assignment if you interview someone with whom you do not have a close relationship. Ideally, one of your “clients” should be over the age of 50. Your project should include all relevant steps regarding the pre-meeting (screening and forms) and first meeting (subjective assessment). Using the tools and concepts outlined in class, gather information about your client, their health history, as well as their physical activity history. Based on the information you gather, determine any contraindications to exercise, barriers to exercise, and any special considerations in terms of health risks. Connect your ideas and writing to peer- reviewed sources or textbook readings if relevant or needed. Writing, grammar, and spelling are expected to be at a 4 th -year University level. Follow APA guidelines (7 th Ed) for in-text citations, references, and appendices. However, you may write in the first person for this project. The project submission should include ONE document (PDF) per client. Submit to Moodle drop box. Each document of the client should have the following: 1. Title Page 2. Client Summary (maximum 2-3 pages double-spaced per client) that includes: o Brief introduction of your client o Results of the interview (discuss any risk factors, and barriers to exercise, and highlight anything important that will guide your decision-making moving forward) o Explain your thought process and what direction you would likely take with each client. Since we have not gone through some of the content for assessment, you do not need to go too in-depth but describe what areas of testing and types of exercises would be important for this client (Ex. Client discussed wanting to increase upper body strength so I want to test upper body and likely include this in the exercise prescription plan). Provide a rationale for your thought processes. 3. SOAP notes collected during the first meeting must be included in an appendix. 4. Pre-meeting forms are filled out and included in an appendix (get-active questionnaire, consent form, and any others you choose to use).

Satisfactory with the organization of the summary paper and the flow of ideas. Some sophistication of analysis is present in the paper and reflection of ideas. Satisfactory sentence structure, spelling, and grammar. Issues with APA guidelines for formatting, citations, references, and appendices. 3 Good - completed forms present in the submission. SOAP notes were organized and written well. Good organization of the summary paper and flow of ideas. Good sophistication of analysis in the paper and reflection of ideas. Good sentence structure, spelling, and grammar. Some issues follow APA guidelines for formatting, citations, references, and appendices. 4 Great – Forms are completed and present in the submission. SOAP notes were organized written well and detail- oriented. Great organization of the summary paper and flow of ideas. Great sophistication of analysis and reflection is present in writing. Great sentence structure, spelling, and grammar. Minor issues following APA guidelines for formatting, citations, references, and appendices. 5 Excellent – forms are completed and present in the submission. SOAP notes were highly organized and extremely detail-oriented based on the interview. Excellent organization of the summary paper and flow of ideas. In-depth sophistication of analysis and reflection. Excellent sentence structure, spelling, and grammar. No issues following APA guidelines for formatting, citations, references, and appendices.
