Eric Dressel Matthew Meyer Khatera Safi Katelyn Humbert Rhonda Kashlan Healthcare FMEA Process Steps 1 and 2 Step 1 . Define the HFMEA Topic (Define your problem narrowly.) Pregnant women who undergo episiotomies or receive perineal lacerations in childbirth do not receive adequate education and teaching. Step 2 . Assemble the Team Team Member Role Rationale for Inclusion 1. Team Leader: RN (You) Throughout the birthing process, the RN is in charge of ensuring the safety of the patient during the procedure, completing proper counts are done and monitoring bleeding. Following the birth, the RN should be providing the patient with education regarding the procedure is the role of the nurse to support laceration recovery after childbirth. Teaching the patient signs and symptoms that occur after a laceration and techniques to alleviate pain should be done by the nurse. 2. OB/GYN The OB/GYN completes the delivery and initiates the episiotomy. As the laceration is being stitched, the OB/GYN should be educating the patient regarding what occurred during the birthing process and what complications can arise as a result. 3. OB tech In addition with the RN, the OB tech checks the patient’s vitals during and following the childbirth. If the OB tech sees any medical errors or inappropriate practices, it is his/her duty to speak up for the patient. Ensuring the patient is comfortable and understands their care also falls under the role of the OB tech. 4. Postpartum RN Following labor, the postpartum nurse monitors the patient’s status and pain resulting from the episiotomy. Prior to discharge, the postpartum RN should teach the patient concerning signs along with expected symptoms as a result of the care she received. 5. Support Person While the nurse educates the patient and provides care, the support person should also learn about signs and symptoms to continue monitoring the patient in the home setting. 6. Charge RN Prior to discharge, the charge RN reviews the patient’s case and ensures
that the patient can safely leave the hospital. This serves as a final check, providing the patient with another opportunity to learn what may bring her back to the hospital. Are all affected or relevant areas represented? YES NO Are different levels and types of knowledge represented on the team? YES NO Step 3 . Create the Graphic (see example based on fall prevention). Use Post-Its on poster or include this as separate attachment if independent assignment . Step 4 . Complete the Hazard Analysis. See HFMEA packet for figures listed below. 1 Sub Process Postpartum RN assesses pt understanding of complications arising from perineal lacerations and provides educational materials where needed. Postpartum RN assesses pt understanding of complications arising from perineal lacerations and provides educational materials where needed. Postpartum RN assesses pt understanding of complications arising from perineal lacerations and provides educational materials where needed. 2 Potential Failure Mode Postpartum assumes pt received proper education prior Poor pt handoff resulting in RN receiving incomplete information RN is distracted/forgetful of assessment due to high pt workload 3 Potential Cause Postpartum RN does poor job of assessing pt’s Lengthy delivery process causes fatigue on team Shortage of RN staff leads to higher RN-pt ratios. Postpartum RNs RN educates pt on laceration complications Laceration occurs RN educates pt on occurrence and what is being done Postpartum RN educates pt on s/s of complications
