You are a CHN in a community where there appears to be an increasing number of obese children in the elementary school that you visit. Describe an upstream approach to community health promotion in this situation. Using the social determinants of health as your framework select three social determinants and identify two questions per determinant you might want to ask and explain the importance of your inquiry. - An upstream approach to counter obesity among children in the community would be through educating the population about adopting the best health practices. Upstream approach aims to shape the underlying determinants of obesity. Therefore, the approach should target areas such as food environments, physical activity environments and the socioeconomic environment. Through seminars, workshops, and schools, I would encourage them to avoid high fatty foods, highly sugary beverages, and use water as drinks to reduce obesity. I would also recommend they engage in the regular vigorous practice for weight management. Such enlightenment could highly motivate the community towards controlling obesity ( Division of Nutrition, Physical Activity, and Obesity, National Center for Chronic Disease Prevention and Health Promotion, 2020). Three social determinants and identify two questions per determinant Education On this issue, I would wish to ask the authorities why there is poor access to education resources in my community. Such lobbying could encourage the government to allocate more resources, including new schools, classes, teachers, etc ., in the community. This could decrease illiteracy, improving overall health. I would also wish to ask the community why they do not take their children to schools. Reasons such as discrimination, lack of funds could pop out, and these could be ideas I could pass to the authorities for such related improvements. This also reduces illiteracy, improving overall health. Employment I would wish to ask the reason for job discrimination in society. Such questions could create shame to those involved, hence reduced discrimination. Improved employment could increase the affordability of care, and controlling issues such as obesity become possible I would also wish to ask why the community people do not have ideas on self- employment. This could be a wake-up call for them, and it could create jobs for both them and their peers. This could also increase care affordability. Infrastructure I would wish to ask the authorities why they fail to improve roads in our community and only improve in other areas. This could encourage road improvement improving care accessibility. Overall care is improved. I would also wish to ask why healthcare providers fail to expand information technology in the community. This could encourage the implementation of technologies such as telehealth increasing care accessibility. Reference

These circumstances are shaped by the distribution of money, power and resources at global, national and local levels. The social determinants of health are mostly responsible for health inequities – the unfair and avoidable differences in health status seen within and between countries.” The root causes of childhood obesity are strongly associated with many of these determinants of health. In Canada in particular, the social determinants of health are largely influenced by economic status (Barnes, 2012) . Children are particularly vulnerable to the environmental conditions associated with poverty, such as food insecurity, inadequate housing, and social marginalization, and there is a clear positive association between socio-economic status and childhood obesity (Barnes, 2012; Mikkenon & Raphael, 2010) . Strategies, programs, and policies that do not acknowledge and address socio-economic status risk the following outcomes: poorer health, a higher likelihood of obesity, and greater social and health inequities among children (Barnes, 2012) . Comprehensive action strategies implemented by health-care providers in collaboration with stakeholders that address the social determinants of health are instrumental to influencing the health of a population and diminishing health inequities Positive changes in the food environment promote healthy eating and, in turn, healthy weights. The expert panel recommends that an assessment of a school’s food environment consider the quality of breakfast programs offered at the school, the food served in the cafeteria, snacks and beverages offered in vending machines, the proximity of other food sources (e.g., restaurants) to the school, and student eating habits and attitudes (Marcus et al., 2009) A school’s physical activity environment includes the playground and recreational facilities, as well as opportunities for physical activity throughout the school day, such as at recess and in extended physical education classes (Charlebois, Gowrinathan, & Waddell, 2012) . Enhanced opportunities for physical activity in the school environment increase the likelihood that children will meet their daily physical activity requirements. Moreover, policies that enforce changes to the school’s physical activity environment in tandem with changes in the food environment promote child health and healthy weights (Phillips et al., 2010) . The expert panel recommends that a school’s physical activity environment be assessed for opportunities for physical activity, including physical education
