1. Pharmacist intervention and counseling efforts during any primary care associated visit will occur with all patients who are on 10 or more medications during such visit 50% of the time within the 1st year of the program 2. Pharmacist intervention and counseling efforts during any primary care associated visit will occur with all patients who have comorbidities/chronic health conditions during such visits 35% of the time within the 1st year of the program. 3. Patients with high comorbities/chronic health conditions, will show a 10% decrease in hospital readmission rates within 1 year after receiving pharmacist related interventions and/or counseling during primary care associated visits for at least 1 year. TARGETED POPULATION ESTABLISHED GOALS Charleston area primary care clinics with patients who meet the following criteria: 1. Any age 2. Are on 10 or more medications (not including over-the-counter) 3. Have a comorbidity or chronic health condition such as diabetes, kidney disease, heart disease, etc. 4. Are at risk for high hospital readmission rates
Month 1 • Identify & meet with all key stakeholders involved in decision making • Assign specific tasks with deadlines, by roles • Identify need for additional AI support & research associated costs • Identify need for additional pharmacy staff & associated costs • Schedule regular, ongoing follow up meetings Month 2 • Review associated costs for additional staff and any additional supporting AI, if needed • Obtain final approval from key stakeholders • Begin key stakeholder involvement in promotion & development of State Law changes Month 3 • Initiate training & education to all staff regarding program & expectations • Begin hiring process • Install additional supportive AI, if required • Analyze current data for targeted population for ongoing monitoring • Determine tool pharmacists will use for medication intervention & counseling TIMELINE AND IMPLEMENTATION MILESTONES
Month 4 • Finalize/hire and train new staff • Implement and test pharmacist intervention tool into EPIC system • Create additional educational & counseling resources in EPIC based upon pharmacist intervention & counseling tool Month 5 • Meet with all key stakeholders to review assigned tasks/projects • Final testing and correcting of any remaining or outstanding issues Month 6 • Go Live TIMELINE AND IMPLEMENTATION MILESTONES
