FMEA is different from RCA because with FMEA (

analysis) it is set up to identify and address possible problems and their effects on the system before an adverse event occurs,

I would say process and design FMEA are not always separate in healthcare because FMEA is a valuable tool for organizations seeking problem-solving techniques and improvements. It enhances system robustness and fault tolerance. Organizations should conduct proactive analysis when adding or changing processes, incorporating patient and resident risk info for proper risk evaluation.

In my organization, I would discuss choosing a topic to analyze with FMEA by analyzing and doing research on the previous events reported and look into what is causing those events and then use the FMEA process to rectify the situation

The FMEA topic that is chose, should be a vulnerable area with a clear and detailed description and the team should consist of high quality workers keeping specific factors in mind when analyzing the process. The
joint commission looks for organizations to do a pre-loss analysis, and before moving forward, they make sure everyone has a strong knowledge
of the criteria being analyzed for the process. The best way to choose a team is to choose people from different departments. h.

The actual version of a process is more permanent, where an ideal process is used for specific purposes/ situations. The actual version of a process is best to use in the FMEA because actual processes identify the ways a process can go wrong and identify the vulnerabilities in that process.This is useful for the FMEA because people use the FMEA to find vulnerabilities in the process, thus helping people create a fault-tolerant system.
