My project is a smaller part of a larger project. Our facility is updating the sepsis protocol
to be in line with current evidence-based practice. My project is to develop the nursing documentation for the sepsis protocol for both the ED and the ICU. This documentation will allow safer transfer of a septic patient from the ED to the ICU and assure continuity of care between the two units. Some of this data can be lost in our facility because the patient is being transferred from the ED which is considered outpatient to the impatient ICU and information and
orders can be delayed if orders are entered under the wrong area. Evidence-based practice is the standard of care for professional organizations like the Critical Care Nurses Association and the American Medical Association which healthcare professionals are held accountable by these organizations and the state professional boards. The sepsis protocol bundle being updated will have a new process with an appropriate documentation to assure labs, fluid resuscitation, antibiotics/antibacterial meds, and other treatments are completed in the correct sequence and timing they need to be completed. Ghost trackers will also be developed to gather the data for compliance tracking.
My facility is a Veterans Administration Medical Center so there are many of the same agencies that we answer to like the Veterans Health Administration (VHA), The Centers for Medicare and Medicaid Services (CMS), and The Joint Commission (TJC). Each of these agencies issue regulations and guidance that must be met in order to maintain our accreditation and reimbursement for services from CMS and insurance companies. Other, non-professional organizations that also provide guidance and accreditation are The National Institute of Health, the American Hospital Association, and the Ohio Hospital Association. This is a big project, but my part will be important to ensure the facility is meeting the standards as put forth by TJC, CMS, and VHA
