C803: Data Analytics and Information Governance
Task 1: Data Types and Structures
Scenario
You are working with other health information management professionals to build data tables for a healthcare organization. Your team has been assigned the job of taking the data elements found in various uniform data sets and building a data dictionary for each uniform data set. You must understand the different types of healthcare data so that you can apply the correct ones to your data tables.
A. Describe the characteristics of the primary data and secondary data that are used in a healthcare setting and include a comparison of how they are similar and different from each other.
A1. Provide a unique example of how each of the categories of healthcare data described in part A can be applied in a healthcare setting.
A2. Describe a relevant potential source of data and how the data could be collected for each application
of healthcare data you described in part A1. A3. Discuss why each type of data is appropriate for the healthcare setting application that you described
in part A1.
B. Discuss an example of how healthcare data from uniform sets can be used in a specified type of healthcare organization.
C. Describe a specific application of data dictionaries within a health information (HI) system.
C1. Justify how the use of data dictionaries could improve an organization’s communication across the continuum of care. Task 2: Data Quality
Scenario
You are an HIM manager in a healthcare organization, and you are responsible for training HIM professionals on the principles of data quality. You will create a presentation to accomplish this training.
A. Create a multimedia presentation (ex. PowerPoint) to train HIM professionals on the importance and principles of data quality.
A1. Include a title slide.
Training on Data Quality
A2. Include a slide that explains how data quality is improved by use of standardized terminologies and classification systems, including presenter notes (suggested length of 1-2 paragraphs of notes).
A3. Include a slide that explains how data quality requirements may be included in organizational policies or processes, including presenter notes (suggested length of 1-2 paragraphs of notes).
A4. Include a slide that conveys how clinical documentation improvement (CDI) and data quality are related, include presenter notes (suggested length of 1-2 paragraphs of notes).
A5. Include a slide that explains why data quality is critical to gathering statistical data (ex. for public health reports, disease registries, or other large-scale needs), including presenter notes (suggested length of 1-2 paragraphs of notes).
A6. Include a slide explaining how to identify data quality problems and their root causes and how to manage or fix them, including presenter notes (suggested length of 1-2 paragraphs of notes).
Task 3: Health Records
A. Compare the characteristics of paper, hybrid, and fully electronic health records.
A1. Discuss issues that may affect the use of hybrid records for legal purposes.
B. Evaluate the attached “Cedar Bend Record Policy” to determine if the policy protects health information for record storage and destruction of paper and electronic health records by doing the following:
B1. Describe whether the Cedar Bend record policy would comply with a chosen state’s regulations, including your reasoning. Identify a specific state in your response. (Note: Refer to the attached “Retention and Destruction of Health Information supporting document to identify your chosen state’s regulations.)
B2. Describe whether the Cedar Bend record policy for retention would comply with the Medicare Conditions of Participation. (Note: Refer to the attached “Retention and Destruction of Health Information” supporting document for a summary of the Medicare Conditions of Participation.)
Task 4: Health Records Documentation and Storage
Scenario
As the chart completion supervisor for Cedar Bend Hospital, you have an upcoming meeting with the HIM director and the chief medical officer (CMO) regarding history and physical (H&P) documentation compliance. Cedar Bend is doing a system update of its electronic health record (EHR), and there are new design capabilities with regard to how H&P assessments are documented in the hospital. There are concerns that stakeholders lack understanding of how the new EHR system should function with regard to H&P documentation and the applicable regulatory requirements. The purpose of the meeting is to make sure Cedar Bend’s policies and procedures that address H&P documentation are current with regard to The Joint Commission (TJC) and state requirements.
A. Create a multimedia presentation (ex. PowerPoint) (suggested length 8-16 slides) for your HIM director and CMO analyzing H&P documentation compliance. (Note: Base your analysis of state regulations on the current state in which you live. If your state does not have specific regulations, you may choose another state that does.)
