Provide a reflection on the simulation and your experience during the simulation scenario today. 1. Give a quick review of your simulation scenario today.
Today my team and I treated a hospice patient who was there for symptom management and pneumonia. Her BP was fine, but RR and O2 saturation was very low. We gave her, her prescribed morphine and she requested something for her anxiety. My team collaboratively decided against giving that medicine at that time because her RR rate was already too low. A member of my team conducted a dressing change on patient’s venous stasis ulcer and patient still had complaints of anxiety, so we were about to give her lorazepam when her RR ceased as well as HR. She was pronounced dead, and Aurora was called.
2. Discuss how you felt going into the simulation experience.
I was very nervous going into this simulation because I didn’t know what to expect. I was also excited to get some hands-on experience with a real-life scenario. 3. Discuss how you believe your performance in the simulation went?
I believe my performance could have been better. I didn’t think about the differences in how to treat a patient that is in comfort care, so I treated it as a typical situation.
4. Did your work assist your team during the experience?
Yes, my work did assist my team. As charge nurse I was able to be a direct line of communication for my team. I was able to contact the healthcare provider about the patients RR and O2 sat and was able to guide my team on the next steps to take. We worked very well together to try and get the best outcome for this patient and provide them with the upmost quality care. 5. What would you have done differently? Reflecting on this simulation, the things I would have done differently are that I would have administered
more prescribed meds. I didn’t want to kill my patient even though she was already dying, she just wanted to be comfortable and that should have been in the forefront of my mind. I also would have asked her if she had any spiritual requests but I didn’t expect her to die on “our watch”. 6. What areas would you like to learn more about after this experience?
I would like to learn more about the different types of care given to different kinds of patients ex. (palliative/hospice vs. ER) 7. How did you feel you supported the patient during the final stages of the simulation?
I feel like I supported this patient by providing them with a dignified death. We tried to make sure that her pain was under control and she wasn’t alone. We could have asked her about her spiritual beliefs as well which she may have liked.
8. How did you feel leaving the simulation about your skills and patient care?
My patient care could use some improvement. I didn’t want to give the patient any meds that were contraindicated with other prescribed meds but in that instance, it was ok because it was just comfort care. I’m glad I paid attention to the contraindications, but it wasn’t beneficial in this scenario
