a dump give out and one of our haul trucks slid down the dump and stood upright. General causes for
this incident were there not being any positive communication between the driver and the dozer
operator. All policies and procedures were not followed by both the haul truck operator and the dozer
operator.

dump is not bermed off, then keep a safe distance and just dump on top. Always have positive
communication with the dozer operator of that dump and know your intentions, do not just assume.
