Introduction

II.A. Accident Investigation Plan
Purpose and Scope: The purpose of this procedure is to deduce the causes that generate accidents through prior knowledge of the events that occurred, in order to be able to design and implement corrective measures aimed at both eliminating the causes to avoid the repetition of the same or similar accidents, How to take advantage of the experience to improve prevention in the company. The registration and control of the accident rate is also the object of this procedure.
The following will be investigated and recorded: ●
All accidents that have caused damage to workers. ●
All accidents with significant material losses or that imply a stoppage of the process. ●
Accidents/incidents that, potentially or by changing some condition, could have had serious consequences, such as outbreaks of fire, free falls of loads, etc. ●
Others that, in the opinion of the direct command, it is convenient to investigate.
Responsibilities:
When an accident occurs in their section or area, direct command is in charge of acting and giving the corresponding instructions to keep the situation under control and prevent further damage. When the accident involves important cures or casualties, you must report what happened, as soon as possible, to the person in charge of your section. The direct command is also responsible for carrying out the investigation of the accidents specified in the scope of this procedure, which occurred in their area or section, and for sending the results of the investigation to management. In the event that the accidents are of a certain importance or that difficulties arise in the investigation of the causes or in the design of the measures to be implemented, you must resort to the advice and cooperation of the person in charge
of the functional unit, the prevention coordinator or a specialist. The directors of the functional units must participate in the investigation when the accidents are serious or could have been, in turn they

must control that the preventive measures agreed upon as a result of the investigated accidents are applied within the established period. The prevention coordinator/prevention service is in charge of advising and assisting in investigations whenever, in a line investigation, the investigator so requires. Likewise, it will carry out the investigation of those cases that, due to their complexity, seriousness, etc., require a specialized investigation. You must also collect accident records and compile accident statistics. The management of the company/Administration must notify the accident to the competent authority and ensure that the measures adopted are complied with. All company workers must collaborate and testify in the accident investigation whenever they can provide relevant information about the event.
Investigation Starting: The investigation will be carried out immediately after the accident once the situation has been controlled within a period not exceeding 48 hours.
All the directors of the functional units where the accidents have occurred must be informed about the measures to be adopted as a result of the investigation. The experiences of accidents at work will be used throughout the company. In this sense, the results of the investigations will be disseminated to the managers and personnel affected by the risks in question. The attached accident and incident investigation form (reference code...) must be completed clearly and in detail to avoid further doubts or interpretations. Each of the sections of the form must be completed by the indicated service or person: Human Resources, Direct Command, Prevention Coordinator, etc.

Statistical Control:
The evolution of the accident rate will be controlled, detecting whether the changes experienced are due to a random fluctuation or a new factor that has modified the safety conditions. For this purpose, the monthly frequency and incidence indices for accidents with sick leave and for total accidents (with and without sick leave) will be calculated. They will be represented according to each month of the year.
Official Notification
Accidents with medical leave The official notification of work accidents (reference code ..........
) will be completed, delivering a copy to the administration department, another to the injured party and the rest to the managing entity ( mutual) within a maximum period of 5 business days from the day of the event. In the case of serious, very serious, fatal accidents or incidents that affect four or more workers, in addition to the previous procedure, a communication will be made (via telegram) to the Provincial Directorate of Labor and Social Security, within a maximum period of 24 hours.
