Samuel has a Specific Intellectual Disability and qualifies for special education services in
the areas of Math and Writing. Due to his Specific intellectual disability, Samuel is not at grade level standards in the areas of Writing and Math. This adversely affects his opportunities to be placed in the general education setting without supports or specially designed instruction. Samuel’s self-help skills include using the restroom independently and using the restroom without assistance. Samuel’s socialization skills include being able start, join and maintain conversations with his peers. However, he does not end a conversation without prompting, and he frequently interrupts others’ conversation. SDI or Specially Designed Instruction means adapting, as appropriate to the needs of an eligible student, the content, methodology, or delivery of instruction to address the unique needs of the student that result from the student’s
disability, and to ensure access of the student to the general curriculum, so that the student can
meet the educational standards that apply to all students. The IEP team determined that the least restrictive environment for Samuel continues to be the resource setting with specially designed instruction in the areas of qualification. Specially Designed Instruction (SDI) will be received via the special education teacher in a specialized math and reading class. Samuel
will have PE in the general education setting. Paragraph #3
List all academic strengths and needs in writing
Examples
Samuel’s writing strengths include: The ability to dictate simple sentences when given a subject.
Samuel still struggles in the area of conventions of writing, especially, when writing simple sentences when given a subject in addition, he still needs the scaffolds, graphic organizers, and editing checklists as guidance and supports for paragraph and essay organization. Otherwise, Samuel struggles to organize his thoughts naturally without these supports. These discrepancies greatly affect his ability to meet grade level standards. Samuel’s writing needs include: The ability to write simple sentences when given a subject. Samuel’s abilities in writing are below grade level and require Specially Designed Instruction. Samuel needs to increase writing skills and the ability to write simple sentences when given a subject in order to decrease the gap between his instructional level and grade level performance. These areas will continue to be goals for his annual IEP. Paragraph #4
List all academic strengths and needs in math
Examples
Samuel’s math strengths include: the ability to do one-digit addition and subtraction problems.
Samuel’s math needs include: two-digit and one-digit without renaming & multiplication
and division. Samuel's independent abilities in math are below grade level and require continued Specially Designed Instruction (SDI). Samuel needs to increase mathematical fluency, computation, and mathematical reasoning. These areas will be his goals for the new IEP.
