Who should get the heart transplant? At County General Hospital, you are on the review board that considers applicants for transplants. A heart has just become available and there are five potential heart transplant recipients. You and your fellow board members must decide which patient will receive the heart. As a group, make a decision on who should get the transplant. Decision: ____________________ Notes:
Patient Files: Adam: Adam is 45 years old, male, has been married for 21 years, and has 2 children (boy 18 and girl 15). He is a research physicist at the University Medical School, working on a cancer immunization project. Current publications indicate that he is on the verge of a significant medical discovery. He has also been a Boy Scout leader for 10 years. Adam’s psychological evaluation reveals that he is distraught about his physical condition, reporting that it interferes with his work. His work seems to place him under a high amount of stress that affects him psychologically. Additionally, Adam’s family relations seemed strained due to his commitment to his work. Adam is a perfect match for the donor heart and has excellent health insurance. Without the heart, Adam is expected to live 6 to 12 more weeks. Bill: Bill is 27 years old, male, has been married for 5 years, and has 1 child (girl 3). His wife is 6 months pregnant. Bill is currently employed as an auto mechanic while he is taking courses in rebuilding automatic transmissions. He plans to open an auto repair shop upon completion of his courses. Bill is not involved in any community service activities. His psychological evaluation reveals that he is strongly devoted to his family and appears to be an excellent husband and father. His capacity for growth in his chosen occupation, however, seems limited. His wife is a trained legal secretary and her prognosis for employment is good, although Bill would prefer her to remain at home to care for the young children. Bill seems unaware of the seriousness of his illness. Bill is perfect match for the donor heart, but does not have health insurance. Without the heart, he is expected to live 6 to 10 weeks. Cora: Cora is 30 years old, female, has been married for 11 years, and has 5 children (boy 10, boy 8, girl 7, girl 5, and girl 4 months). Cora is a high school graduate, but has never been employed. Cora and her husband have just purchased a home in the suburbs and Cora is planning the interior to determine whether she has the talent to return to school for courses in interior decorating. She is also a member of several religious organizations. Cora’s psychological evaluation reveals that Cora seems to be able to talk about nothing but her religion and children. She seems resigned to her illness and likely death. Her husband is self-employed, works long hours, and is in good health. Cora’s mother lives with the family and handles a large portion of the childcare. Cora is a perfect match for the donor heart and has decent health insurance. Without the heart, she is expected to live 8 to 12 weeks. David: David is 19 years old, male, and single. He is currently a sophomore at a large eastern university majoring in philosophy and literature. Eventually, he hopes to earn his Ph.D. and become a college professor. David is actively involved in various campus political organizations and – as an outspoken critic of college administration – was once briefly suspended for protesting. David’s mother is deceased, his father works as a lawyer, and he has 2 sisters (ages 15 and 11). His psychological evaluation reveals that David is highly intelligent, appears confused about his future, and demonstrates a penchant for jeopardizing it by involving himself in various risky causes. David’s father has invested a lot of time and money in hopes that David would become a lawyer. Father and son relations are strained and David appears to only be mildly concerned about his sisters. David is bitter about his illness. He is a perfect match for the donor heart and has excellent health insurance. Without the heart, he is expected to live 6 to 8 weeks. Anna: Anna is 34 years old, female, and single. She is currently employed as an executive at a large manufacturing company where she has worked since college graduation. Anna is a member of a local choral group and was the alto soloist in a Christmas musical. She has been very active in several church and charitable groups. Anna’s psychological evaluation reveals that she is a self-contained, inner-directed woman, and a motivated team player. She shows some combative tendencies. Her employers regard her as indispensable with a superb work record. Anna has also been effective in her charity work. She is well regarded by all who know her, although she seems to have few, if any, close friends. Her parents are deceased, but she has 1 brother (age 40), and a nephew (age 10). Anna is a perfect match for the heart and excellent health insurance. Without the heart, she is expected to live 10 to 12 week.
