Unit 1 Assignment 1 - Hospitals, Stakeholders & Medical Care 1. Identify the 3 reasons hospitals merged or were consolidated. Discuss the impact these actions had on cost for the organizations. (One paragraph is an appropriate length.) The three reasons hospitals merged or were consolidated are lower reimbursements, increased capital, and eager health systems. These come at a cost that impacts significantly. In smaller nonprofit organizations, some capital needs are being met. To gain more control over the costs of care, these organizations merge with each other. This merging trend is becoming popular as it allows for better alignment with reimbursements incentives for population-based health outcomes and creates new relationships between different components of healthcare delivery. Systems of reimbursement are anticipated to persist. 2. What is meant by the phrase “Healthcare Stakeholders”? Identify 3 major stakeholders and describe their role in the healthcare system. (One paragraph is an appropriate length.) The phrase “Healthcare Stakeholders” are persons or groups that have a vested interest in a clinical decision and the evidence that supports that decision. Three major stakeholders are the public, the employers, and the providers. Employers are also major stakeholders; they pay a high proportion of the cost and take proactive roles in determining what those cost should be. Providers are the core of the industry; they have a lot to do with the actual process and outcomes of the services provided. 3. Differentiate the usefulness of outcomes-based research and random clinical trials (RTC) in terms of its application to real-world scenarios. (One paragraph is an appropriate length.) A solid grasp of research-based outcomes is crucial in understanding the growth, development, structure, and function of the human body as well as how it reacts to external factors. These types of outcomes are vital for progress within clinical medicine. Clinical research mainly concentrates on medical care procedures. Both the randomized clinical trials and research-based studies must be utilized together in real-life scenarios to advance treatments, diagnosis methods and preventative measures effectively. 4. Compare and contrast two trends of U.S. health status indicators and costs as compared to other developed countries. (One paragraph in an appropriate length.) Two trends of U.S. health status indicators include general mortality and life expectancy. The general mortality rate in 2018 was 723.6. The life expectancy for the U.S. population is 78.7 years in 2018 ( Mortality in the United States, 2020). In 2014, Ireland had 29,095 deaths ( Deaths - CSO - Central Statistics Office , 2016). For the year 2021, the life expectancy in Ireland is 82.51 years of age ( Ireland Life Expectancy 1950-2020 , n.d.). The life expectancy in Ireland is obviously longer than that of the United States. The United States also has a larger general mortality rate than Ireland. This could be due to the United States having a larger population.
5. Avedis Donabedian identified three basic components of medical care - structure, process, and outcome. Explain what each component is. Evaluate these elements with regard to the impact of hospital consolidations and mergers on access to care. (One paragraph is an appropriate length.) You must cite at least one credible source in addition to your textbook. Cite your source using APA. “Donabedian suggested that the number, kinds, and skills of the providers, as well as the adequacy of their physical resources and the manner in which they perform appropriate procedures should, in the aggregate, influence the quality of subsequent outcomes (Johnson et al., 2022).” The first basic component of healthcare is structure, and this is the physical and organizational components where healthcare occurs.  The second basic component is process. This component includes the actions and the way they are carried out. An example of this would be interactions between patients and providers. The third basic component is people, referring to both patients and staff members involved in healthcare services. In a healthcare setting, there are organizational elements that facilitate the provision of medical care. Donabedian believed that structure measures have an effect on process measures, which in turn affect outcome measures. Together these form the basis of what is required for an effective suite of measures (UNC School of Medicine, n.d.). 6. It also depends on if they have enough resources to do their job well, and if they perform procedures correctly (Johnson et al., 2022). This component includes the end result of the care and the experience that the patient had as well. With a larger number of medical professionals available within the same area, merging hospitals not only lead to lowered costs for patients but also potentially results in better quality care based on individual preferences.
