Patient-Centered Medical Home The patient-centered medical home (PCMH) model is an approach to help deliver high-quality, cost- effective primary care (Centers for Disease Control and Prevention, 2021). This advanced primary care model has been associated with the goal of improving patient health outcomes by providing resources needed to effectively manage chronic diseases (Almalki et al., 2018). With a large and growing community, the PCHM model was developed to help patients become more involved in their care and work with the provider to develop a care plan to help manage their chronic disease(s). An advantage the PCHM model provides is the ability to obtain accessible services in a short amount of time. According to the Agency for Healthcare Research and Quality (2022), the accessible services available to patients allow for shorter waiting times for urgent needs, enhanced in-person hours, around- the-clock telephone, or electronic access to a member of the care team, and alternative methods of communication such as email. PCHM is based on patient-centered care and the ability for a patient to have access to their care team allows for fewer visits to the emergency departments or urgent care.
