Barbell Prescription Medical Conditions 1 2023 Purpose: The primary purpose of this assignment is to create a bridge from applied physiology to medical conditions. Risk factors for cardiovascular disease and the implications of systemic inflammation were previously introduced. The chapters in this book compare/contrast the lifelong implications of lifestyle on cardiovascular disease. Specifically, the book advocates for resistance training over physical activity. The secondary purpose is to determine the implications of this information for individuals across the lifespan and determine how physical therapists can utilize this information effectively to improve the health and wellness of society through primary, secondary, and tertiary prevention. Instructions: 1. Read the first 6 chapters of Barbell Prescription: Resistance Training after age 40. 2. Briefly answer questions #1-8 and submit to D2L by Jan 18 th . 3. Post your answer to #8 on the discussion board and comment on at least 2 other posts. 1. Compare/contrast the following terms as explained in the text and explain the significance: exercise, training, and practice The book differentiates the three terms exercise, training, and practice, which are often conflated by the general population. Exercise is any physical activity that goes above the basic tasks of daily living. It includes any activities such as walking the dog, playing tennis, washing the car etc. Exercise is the broader of the three terms. Training is targeted exercise that aims to achieve a specific fitness outcome. Training accomplishes this by manipulating training variables as a part of a long-term program that aims to increase one or more of the general fitness attributes. 2. What are the General Fitness Attributes? The general fitness attributes are as follows 1. Strength 2. Power 3. Endurance 4. Balance 5. Mobility 6. Body composition 3. What is the sport that the master athlete is competing in? Explain the relationship between “compression of morbidity” and being a master athlete.

The best example of compression of morbidity as it relates to the extreme sport of aging is seen in the example Dr. Sullivan paints with Phat Phil and Well Will. Due to his healthy lifestyle and participation in activities that actively countered development of metabolic syndrome, Will was active well into his older age, and died suddenly/painlessly. Phil, on the other hand, suffered years of metabolic syndrome, adverse cardiovascular events, and other morbidities associated with his poor lifestyle and health. Will effectively “compressed” his morbidity to an almost nonexistent degree, while Phil experienced significant morbidity in the last two decades of his life. 4. Chapters 4 & 5 were covered in Applied Physiology but presented in a new way. Was it all review, or are there specific concepts you now understand better? Regarding the physiological principles, it was all review. However, I did enjoy the new perspective Dr. Sullivan offered by painting a picture of “human apoptosis” at the macro-level. I am very familiar with cellular apoptosis but had never thought of an unhealthy lifestyle as “your own body eating itself”, but it makes perfect sense. 5. Do you agree that resistance training is the best "general conditioning activity" for older adults? Did the book change the way you think of strength training in the elderly? For older adults, I would agree. It seems to posit enough hard evidence that resistance training (combined with some form of HIIT if possible) is the ideal regimen for older adults in order to decrease risk of developing metabolic syndrome and to increase QOL. Prior to reading the book, I knew that resistance training was much better for older adults than is generally believed, but I had no idea the magnitude of change it could affect. 6. How does the concept of physiologic 401K relate to your role as a PT? As a middle-aged patient’s PT, with the knowledge I now know regarding physiologic 401k and the role resistance exercise plays in aging, I feel it is my duty to incorporate that into patient education. There is nothing I can do to make my patients be healthy, but I can do my best to emphasize and help them understand the gravity of the “sport” they are competing in, and get them to reconceptualize themselves as athletes. This is personally relevant to me, because my dad is 57 and Is staring some of the more grim prospects of metabolic syndrome in the face. I’ve already had him read a few chapters from the book and sat down to talk with him about it. Because I can’t idly let my dad fall into decades of suffering when there are concrete steps he can take to improve his quality of life.
