Look up the following terms and provide the codes for SNOMED-CT and ICD-10, and provide a description of the information that was observed while determining the codes. What do you find when mapping the codes? I searched for “hemangioendothelioma” and “Composite hemangioendothelioma’ was an option that was listed among others. 2. Second-degree burn of foot, initial encounter a. SNOMED-CT - 62537000 b. ICD-10 – T25.229A Burn of second degree of unspecified foot (initial encounter) c. What do you find when mapping the codes? I first tried to search for “Second-Degree burn of foot” and it gave me a very long list some of which have nothing to do with foot burn. Then I search for “foot Burn” it narrowed the return, but it was still long. Then I searched on the internet if there is another name for Second-degree burn, and I learned it was also known as “partial thickness burns”. Once I got that, I chose the option of “partial thickness burn of the foot. Since it is not specified the side or part of the foot, I picked the option without any specifications. Once I chose that, it gave me the option to choose the episode refinement and I chose “initial encounter” 3. Sleep apnea a. SNOMED-CT - 73430006 b. ICD-10 - G47.30 Sleep apnea, unspecified c. What do you find when mapping the codes? I typed the word “sleep Apnea” and I got lots of options but the first option that came was “sleep apnea” with no additional specification and that was exactly what I wanted in this case. 4. Herniated urinary bladder, male, 54 years old a. SNOMED-CT – 410070006 b. ICD-10 – G32.89 Other Specified disorders of bladder c. What do you find when mapping the codes? I searched for “Herniated Bladder” and found “Herniated Urinary Bladder”. I also tried to add the gender and age of the patient but it didn’t give me a different code. 5. Atrophic scar

I searched for the exact word that was given and it gave me multiple options. The reason I picked this code is there was no additional specifications were given for this exercise. 6. For this one, I searched for carcinoma in situ breast. From the options, I chose “carcinoma in situ of breast” as there is no need to specify for this assignment. 7. Mild depression a. SNOMED-CT - 310495003 b. ICD-10 – F32.A Depression, unspecified c. What do you find when mapping the codes? I search for “mild depression” and I found the exact match with no specifications. 8. Failure to gain weight a. SNOMED-CT - 36440009 b. ICD-10 -R62.51 c. What do you find when mapping the codes? I searched for “failure to gain weight” and found an exact match. Answer the following: 1. What does SNOMED mean? The exact meaning is Systemized Nomenclature of Medicine. It is a uniform multilingual terminology of medicinal terms that is used around the globe. According to National Library, “The terminology is comprised of concepts; descriptions, which contain terms that explain the concepts; and relationships, which define the type of the association between two related concepts.” 2. Why is it important? The importance of SNOMED is to benefit all aspects of healthcare and assists evidence based care. If we use it in the electronic medical records software, it will be applied to give uniformity within the record. It will also prevent confusion that may be produced by using local languages and slang to recognize an idea.
