passed House Bill 1 detailing the
regulations of pain clinics and prescription
drug abuse in Kentucky. The regulation
201 KAR 9:260 sets mandatory guidelines
for prescribing and dispensing controlled

KY Office of Drug Control Policy (KY-
ODCP) and KY Agency for Substance
Abuse Policy (KY-ASAP) are tasked to
coordinate KY’s response to substance
misuse and abuse. The goal of these
offices is to plan for development for
an effective continuum of prevention
and treatment services. Another goal

The Centers for Disease Control and Prevention clinical
practice guidelines for prescribing opioids for pain were
created to help clinicians make informed decisions when
prescribing opioids. These guidelines are intended to
improve pain management and patient safety. The Centers
for Disease Control and Preventions recommends clinicians
use the following actions when prescribing opioids:
determining weather or not to use opioids for pain
management, selecting appropriate opioids and determining
the correct dose, deciding duration of prescription, and
conducting a follow up, and assessing risks and potential
harms of opioid use.

The 2022 Centers for Disease Control and Prevention clinical practice guidelines is intended for
clinicians to use when prescribing opioids for acute (less than 1 month) and subacute (1 to 3 months)
pain for adults in outpatient settings aside from those patients with sickle cell disease, cancer related
pain, palliative care, and end-of-life care (Dowell et. al., 2022). Opioid therapy for acute pain should
only be initiated if its benefits outweigh its risks (Dowell et. al., 2022). Opioids should be prescribed at
the lowest effective dose and for a duration no longer than the pain that warrants opioids lasts (Dowell
et. al., 2022). If opioids are being used for more than a few days, tapering doses are recommended
when discontinuing opioids (Dowell et. al., 2022).

The APRN must have a collaborative agreement for the
advanced practice registered nurses prescriptive authority
for controlled substances (CAPA-CS). The APRN must also
register with the DEA and obtain a DEA number and register
with KASPER.

history including history of substance abuse and a physical
examination of the patient in the patients medical record.
The APRN is also required to discuss with the patient or
legal guardian about the risks and benefits of controlled
substances including the risk for dependance and that the
controlled substance will be discontinued when the requiring
condition has resolved. This discussion is required to be
documented in the patients medical record.
