Unit 6 Discussion PCS Code Scenario: Treatment Performed: Lymphatic pump of lower extremities for alleviation of cellulitis and edema. Indications: A 57-year-old female patient with a history of type two diabetes militias and neuropathy presented today for a follow up check of mild cellulitis bilaterally on both lower limbs. Patient has continued treatment with oral antibiotics for the past 10 days. The patient presents today with mild edema bilaterally and a long-term, low-dose antibiotic was also prescribed for an additional treatment of cellulitis. It was recommended that the patient have osteopathic lymphatic pump treatments to help reduce lymphatic fluid in the lower limbs. Treatment Report: Patient was placed in the supine position; the physician placed moderate pressure to the patients’ feet on the plantar surface. A pedal pump technique was used, and flexion of the feet was performed for 2 minutes in a pump motion approximately 100 times, then the physician placed moderate pressure to the patients’ feet on the dorsal side for an additional 2 minutes in a pump motion approximately 100 times. This cycle was repeated 4 times. I did get the same code with the code book and the 3M encoder, the process for getting the codes differ slightly. The only root operation in the Osteopathic section is “Treatment”, in the code book I started in the index under “Osteopathic Treatments”, then followed with “Extremity”, “Lower”, and that gave me the first 5 characters of the treatment code 7W06X. When using the 3M encoder, I selected the product “Codefinder” and put in the age of the patient, selected “add procedure” and entered the keyword “Treatment”. This gave 5 options and “Treatment, osteopathic” was what I was looking for. The next step was “Treatment, External” and I had to select what method was used representing the 6 th character value, this being “Lymphatic Pump”. The next step was to select body region representing the 4 th character value “Lower extremities” this gave me the code 7W06X6Z Osteopathic Treatment of Lower Extremities using Lymphatic Pump. The process is the same if you identify the main term or keyword which represent the root operation. If you understand the root operation, you are likely to get the same result using either the code book or the 3M encoder. I prefer to start with my code book, because I know on my CPC exam that will be all that I am allowed to use. After I find the code in my code book, I like to check the code with the 3M encoder.
