2 Ethical Dilemma in Adolescent Therapy: Balancing Confidentiality and the Duty to Inform. Introduction: Practitioners in social work frequently face complex ethical quandaries that necessitate careful analysis and adherence to professional norms. The Ethical, Thematic, Holistic, Individual, and Contextual (ETHIC) Model provides a framework for ethical decision-making. The ETHIC Model will be used in this essay to respond to a practice scenario featuring a 15- year-old female client with an eating disorder who confesses her pregnancy and requests confidentiality. Communication with the Client: When dealing with the client's pregnancy reveal, the ethical ideal of secrecy should be balanced against the potential harm to the client or others. The National Association of Social Workers (NASW) Code of Ethics emphasizes the significance of maintaining client privacy, except when there is a reasonable fear of urgent risk to the client or others, according to norm 1.07(b). The social worker must engage the client in a collaborative and therapeutic discourse in this scenario. The practitioner should explain the boundaries of confidentiality, emphasizing the responsibility to notify parents if the client's safety is jeopardized. However, the social worker should consider other options, such as involving a trusted adult, to limit potential harm to the client while preserving her autonomy. Communication with the Client's Mother:  My job as the social worker is to balance transparency and confidentiality if the client's mother inquires about the session after it has concluded. Social workers should explain the nature and boundaries of confidentiality with clients, including the involvement of parents or
3 guardians, according to NASW guideline 1.07(c). In this case, the social worker can inform the mother that, due to the therapeutic process's confidentiality, specifics about the session cannot be released without the client's explicit agreement. On the other hand, the social worker should encourage open communication between the client and her mother, emphasizing the necessity of assistance during tough times. Justification: The NASW ethical guidelines emphasize the importance of client autonomy, privacy, and safety. The tricky balance between respecting confidentiality and intervening where there is a reasonable suspicion of harm is acknowledged in Standard 1.07(b). The social worker conforms to the ethical concept of transparency while prioritizing the client's well-being by engaging in open discussion with the client regarding the limitations of privacy. The ETHIC Model Applied. E - Examining relevant personal, societal, agency, client, and professional values: In this situation, it was important to consider the values of confidentiality, autonomy, and the well-being of the client and her unborn child. The protection of minors and the significance of parental engagement in their lives are generally valued by society. It is my responsibility as a professional to put the client's best interests first while also taking legal and ethical requirements into account. The client values her privacy and autonomy, and she explicitly asks you not to share the information with her parents.
