Client Strengths Describe the client’s strengths (personality traits, beliefs, skills, integrity, work ethic, etc.) Claireece displays an introverted and timid personality type due to her trauma. This causes her to question the intentions of people whilst avoiding many interactions with teachers and peers. Despite Claireece’s hardships she Is a determined and intelligent teen who has hopes to turn her life around and finish school. 2. Race, Gender, Culture Describe how the client identifies in relation to race, ethnicity, gender identity, and sexual orientation. Address any specific cultural influences or practices, and any known issues relating to others who identify the same or differently. Claireece is an African American presumably heterosexual female. Specific cultural influences include claireece growing up in the 134 th street district of Harlem, New York which is a high poverty area. Culturally claireece grew up in survival mode. 3. Religious/Other Faith Describe whether the client identifies with any specific religious or secular faith or has indicated agnostic or atheist preferences. Address any impact that faith in the family of origin has had on the client. Claireece never explicitly mentions her religious beliefs, however, the Christian religion is what is being displayed throughout the film. 4. Social/Emotional/Moral/Cognitive Development Identify where the client falls in each of the following stages of development. State what typical development looks like at that stage and describe the client in relation to each (use examples), explaining whether the client appears to be on target for each stage related to his/her chronological age. a. Psychosocial Claireece is in the identity vs. confusion stage of her psychosocial development which is normal for someone her age. Claireece is struggling with things regarding appearance, education, relationships, sexuality, social views, personality, and interest. She is trying to grasp a concept of self as well as the world around her and what she wants to embody on her path. b. Claireece is beginning to use her intellectual abilities and applying It to abstract ideas, but she is still slightly behind children her age. c. Moral Morally, Claireece is in the conventional stage of this form of development. Claireece displays a “good girl” morality and conforms to avoid disapproval and dislike by others, especially her mother. She is slightly behind in development for this stage. 5. Physical Health Discuss any known information about the client’s physical health as listed below. (Note, mental health will be addressed in a future assessment). a. Prenatal Health and Exposure to Drugs and Alcohol in Utero Prenatal health and exposure to drugs or alcohol in utero history currently unknown. b. Chronic Conditions/Disabilities Chronic Conditions/disabilities of client currently unknown. c. Major Illnesses Client is HIV positive. d. Accidents or Hospitalizations Client was hospitalized for a couple weeks for the birth of her second child. e. Allergies History of allergies currently unknown. f. Weight/Growth Issues Claireece is overweight. g. Medications Taken History of medications taken currently unknown. h. Growth Comparison to Same-Age Peers Claireece is morbidity obese which is not normal for a child her age. 6. Substance Use Describe any known or suspected use of legal or illegal substances (cigarettes, alcohol, medical or legal marijuana, prescription, or illicit drugs. List the age of first use, frequency (i.e. daily), and intensity (i.e. 2 packs of cigarettes daily). History of suspected use of legal or illegal substances currently unknown at this time. 7. Education/Vocation a. Describe the client’s educational background, including highest grade completed, special education status, enrollment history (i.e. multiple elementary schools), any truancy issues, major academic upsets, and academic performance. HMSV 448 Interventions & Advocacy with Children Claireece has a challenging academic performance initially, however, after being kicked out of her middle school she is enrolled in the each one teach one program. This program is described as a GED readiness program and prepares students to sit for the GED exam. There is where Claireece discovers her love for reading and writing and even starts reading to her newborn son. b. Describe the client’s work history, training, work skills, periods of unemployment, major employment events and performance. Client’s work history and performance is currently unknown. 8. Interests/Hobbies Describe any specific extracurricular interests of the client. Client likes to read and write. 9. Living Arrangements Describe who the client was parented by, who lived in the home while growing up, the client’s housing history, and current living situation. Claireece lived in the home with both parents in her earlier childhood then transitioned to living with her mother alone in her early teen years. Currently Claireece no longer lives with her mother and is getting housing assistance. 10. Family of Origin/Upbringing Describe as much as is known about the client’s biological and any other substitute family or caretakers. Be sure to identify both “kin” (biological/legal relatives) and “kith” (those not biologically/legally related but either indirectly related (i.e. the biological parent of a half-sibling) or considered as family by the client (i.e. very close friends). Note, this should cover everyone in this category whether the client is in contact or on good terms with anyone. Claireece’s mother, Mary Johnston, is unemployed and relies entirely on welfare to support both Claireece and her. She is a very isolated woman and is described by claireece as not leaving the house often or having company over. Claireece says her mother spends most of her time sleeping, eating, watching television, and engaging in abusing behavior towards claireece such as, throwing things, screaming, yelling, fighting etc. In the home. Claireece’s father, Carl Jones, is not directly present in the film but used to live with Claireece and Mary. During this time Carl was sexually abusing Claireece in the home and is the biological father to both of her children. Claireece’s grandmother, Toosie, is the guardian of Claireece’s daughter “Mongo”. 11. Family Strengths + Areas of Dysfunction Describe any family strengths that may balance out present dysfunction. Claireece appears to have a better relationship with her grandmother toosie. Describe maladaptive family relationships, patterns, conflicts, or behaviors that impact the client. The biggest conflicts within this family that directly affect the client is the physical, emotional, psychological, and sexual abuse from both parents in the home for an extensive period of her adolescent years. 12.
