SWK-516 Multidimensional Assessment Case Study Worksheet Biophysical Psychological Jean Case Study: Her biophysical factors include not eating and not sleeping. She is also hearing voices and having religious delusions. She believes that other students at her college are trying to ruin her life because of her special powers. Jean is feeling overwhelmed, and she is socially isolated. She is having delusional thoughts which are making her restless. She believes she has superpowers. Jean talks to her mother about how the world is ending and even tells the social worker that God told her the end is near. Cassandra Case Study: Cassandra feels depressed and can’t take her medication properly. She is dealing with changes to her body and appearance. There is the need to feel accepted by her father, experiences of anxiety & fear. She also feels overwhelmed and unsafe. Sharon Case Study: Has sickle cell anemia and struggles to take care of herself and take her medication regularly. Has trauma and anger due to her daughter being molested by her ex-boyfriend. Verbal and emotional abuse from her family. Charles Case Study: No longer interested in church, falling behind in school, feeling like he doesn’t fit in with other kids and feels isolated. Depression, anxiety, and feelings of isolation. Carly Case Study: The sexual assault committed by her ex-boyfriend 4 years ago and her herpes diagnosis from that experience. Fear of being in a committed relationship due to not being able to share her diagnosis, also feels shame and hopelessness. 1. I chose Sharon who struggles with trauma and emotional abuse. Culture can highly influence an individual’s decision to leave an abusive relationship. Sharon is African American and often within this community going to seek professional help is not encouraged. Her parents are very emotionally abusive and while they are financially supportive, they don’t help Sharon improve her physical health. Culture can have a significant impact on how you face certain situations in your life and for Sharon her family seems to face situations by being open about traumatic experiences. Instead they are dismissive and unsupportive. 2. I think the most serious biophysical issue for Sharon is struggling to take care of herself. She has been diagnosed with sickle cell anemia and has made several trips to the hospital preventing her from getting a job. She wants to become financially independent from her parents to provide for herself and her daughter. However, because she does not prioritize taking her medication regularly it will be difficult to
accomplish this. She has recognized this cycle she is in and wants to break it. 3. Sharon could seek help from a social worker who could help her get access to the proper resources. A social worker could assist her with setting routines that prioritize self-care and include making time to take her medications. By working on setting an easy-to-follow routine Sharon will likely begin to feel better about herself and she will be able to get a job so that she can provide for her daughter. 4. For the psychological factor, Sharon struggles with depression, trauma, and anger. She is a single mother and because of her depression she struggles to take care of herself. When experiencing a depressive state Sharon must remember to keep up with her treatments and medications. By doing this she is ensuring that she is taking care of herself, and she will be able to handle her other obligations such as taking care of her daughter. 5. For Sharon some of the environmental factors that may affect her would be not having the proper support system. It is helpful to have someone to talk to when you are dealing with difficult situations. Sharon lives with her parents who are emotionally abusive towards her. They provide financially for Sharon and her daughter, but they blame Sharon for everything that happened to her daughter. In the client’s situation, it may be that she needs a support system because she is sick from her illness, or she just simply needs help. As the case manager I would address the situation by trying to find her resources that would make dealing with her illness easier. For example, access to childcare, assistance with bills, or having a therapist to talk to about her anxiety and trauma. I can help her find a support system by finding groups or centers of people who are going through similar experiences. This will make her feel like she is not alone and has multiple people to talk to about her struggles.
