Presenting Problem: Client is a 17-year-old male, been in the CPS system for majority of his life, current case is based off of his own drug use and getting treatment before he ages out of the system. Background information: Client is a 17-year-old male who has been familiar with CPS for many years and has been officially placed with his grandmother for 3 years though she had been the primary caregiver for years prior. Placement with grandmother was because of mother’s inability to care for her children because of her drug addiction. Both mother and sons’ drug of choice is Methamphetamine. The client did not come forward himself, but his grandmother asked CPS for help in raising a 17-year-old boy with drug addiction, CPS was able to help because they did have an open case because of the placement with the grandmother. Client had already been in trouble with school and had been in alternative night school for the past 2 years, though no improvement in grades has happened since starting alternative school. Impressions and Recommendations: Client is a 17-year-old male, been in the CPS system for majority of his life, current case is based off of his own drug use and getting treatment before he ages out of the system. The Clients drug addiction is not immediately evident in appearance or attitude, with clean appropriate clothing and a positive attitude for the situation The Client appears like any other teenager. The Client is very reserved and soft spoken. Client was nervous but can also be explained by the setting of a doctor’s office for a COVID test. Client has strong connection with grandmother and seeks her for comfort when nervous. The agency, CPS, has offered The Client an opportunity to attend of group home/rehab out of our area to help him with his drug addiction and hopefully get him started on the right track before aging out of the system. The Client accepted the offered of the group home/ rehab and will do well if he actually takes advantage of the services offered there. The Clients grandmother has shown she is a great support system for The Client and will continue to be there for him even after he ages out of the system. Treatment Plan: Goal 1: To achieve sobriety Objective 1: Go 1 month without drug use Objective 2: Go 3 months without drug use Goal 2: To graduate or attain GED Objective 1: Improve grades to C or better in 2 months or less Objective 2: Learn what is offered at local community college for possible career paths.
Contact Note: D- Client spent most of the visit wringing his hands and in the car with his grandmother. He only spoke when spoken to. Despite his nervousness I was able to get him to talk a little bit about school and his tattoos. When first offered the option of me coming in the doctor’s exam room with him he nodded it was fine, I felt his hesitation and told him that he was allowed to tell me no if he did not want me in there. He very quietly told me that he’d rather me wait in the waiting room. I believe this interaction led to better future interactions with him because he knew I was not there to control him and that he does have a voice when working with me. A- The Client was at a doctor’s office for a COVID test with a new face for him (Me) so his nervousness and demeanor were appropriate for the situation. The client was open with both me and the doctor about their recreational drug use but did not admit to cigarette smoking, although the smell of cigarettes was very strong on him. Overall, the client is in a stressful transition to improve his life and is handling it quite well, the nervousness and apprehension are all normal responses to the changes in his life. P- The Client is going to attend and use the group home/rehab’s resources to improve his life and achieve sobriety. Client will continue to work towards goals and objectives as he transitions into the adult world. Client will utilize his support system to achieve his goals.
