The client voluntarily requested a psychiatric examination due to the observation of diminished energy levels and persistent exhaustion lasting about four weeks. She has expressed a decline in her inclination to engage in social activities and carry out her regular daily tasks, such as physical exercise. She is experiencing doubt over her career decision and is struggling to maintain focus on her tasks. A psychological evaluation was requested to get more information on Barbara B.'s current level of functioning. Barbara B. voluntarily sought assessment, expressing that she has been experiencing fatigue and a lack of energy for around four weeks. She has expressed a decline in her inclination to engage in social activities and carry out regular tasks, such as physical exercise. She doubts her career decision. We have obtained the client's informed consent in accordance with Area 8.02 of the American Psychological Association's Code of Conduct and Moral Standards for Psychologists. According to Section 8.02 of the American Psychological Association's Ethical Principles of Psychologists and Code of Conduct, the client has been informed of all her rights. The client has been informed about the factors that might influence her willingness to participate, including any risks, emotional distress, or negative consequences (American Psychological Association, 2017).
Adhering to State Laws and the guidelines set forth by the American Psychological Association regarding Informed Consent, the initial session with the client encompassed the act of providing the individual with information regarding the potential risks, benefits, and anticipated results of their voluntary participation in a research project, medication, medical procedure, or therapeutic approach. The user has been informed about the main purpose of the psychological services, the boundaries of confidentiality, and has been notified that the final psychological report will include personal information, intervention and treatment recommendations, and the clinical assessment of the medical professional involved. Observations The client, Barbara B., has sought psychological services at Lifestance Mental Health Clinic under the care of Dr. Mahletta Abott, a Consultant Psychiatrist. This psychological report contains sensitive information regarding the client's psychological evaluation, which was conducted to address concerns regarding her social, emotional, and behavioral functioning. Data Barbara mentioned that she had to rush to her appointment after sleeping in and missing breakfast that morning. When discussing her past and present concerns, she displayed a genuine and honest demeanor. She approached the testing procedure with caution, ensuring she understood each step and seeking clarification when needed during the projective tests. She displayed an elevated level of professionalism and empathy while administering the Wechsler Adult Intelligence Scale. Although she completed fewer assignments, her answers were consistently accurate. Her non-verbal duties were executed with exceptional precision, resulting in significant outcomes.
