Definition of Disorder and Summary of Major Features Dissociative identity disorder is a mental disorder when a patient has two or more personalities. It is formerly known as multiple personally disorder. It’s characterized by “switching” to alternate identities. This person may feel as if there are multiple people living in their heads. Some people report feeling possessed. Every identity possibly has unique characteristics such as their name, age, gender, mannerisms, and even some physical qualities. A person suffering from the associative identity disorder is also likely to suffer from this sociative amnesia and/or dissociative fugue. B. Case Example Here I have a case study of a 55-year-old Caucasian woman who presented to the hospital with reports that she had been constantly dominated by her alternate personalities. She stated that she only learned of them being there because people told her, most often after an incident. She also reports that stress in the use of substances make the fragmentation of her personality worse. She is plagued by three other personalities, of whom one is a 7- year-old child, a teenager, and a male person. There is no family dynamic, or economic issues reported. During the “switching” the patient becomes extremely violent. This ranges from homicidal to suicidal thoughts actions feelings and behaviors. Due to this, she has been hospitalized in a mental facility. Treatment for this woman included psychotherapy (talk therapy) with cognitive behavioral therapy (CBT) to address her stress and use of substances. Psychotherapeutic treatment was for six months and was also paired where drug therapy. She was prescribed Escitalopram (Lexapro) in order to reduce her level of anxiety. Although the patient feels that the medication is helping, and that her condition is getting better, after six months it in fact had not and the treatment continues.
Reading Head: Lab 1 Assignment Submission Link: PSYT 2331 3 C. Mental Status Exam on Case Example Based on the mental status exam the patient appears to have a few odd physical characteristics. She is restless fidgety and panicked. She also appears guarded, becoming hostile and challenging when “switching”, either turning homicidal or suicidal. Patient is speaking loudly but not continuously. There appears to be some loose association, but no auditory or visual hallucinations are present. However, feelings of anger, anxiety, and some irritability is. Effect is neither blunted nor elated. It is appropriate to thoughts. The patient does seem to be of high intellectual function without deviation. She is oriented times one. Her sensorium is not clear because the only thing that she can be sure of that she went to the hospital in search of help. But due to her constant “switching” she seems to be having a hard time keeping up with the people she comes into contact with and how much time has passed in between. D. DSM-V Criteria on Case Example Based on the DSM-V criteria, the patient is exhibiting four of the five criteria for a diagnosis of dissociative identity disorder. The report of her feeling as if she’s been dominated or taken over by multiple alternate personalities is the first indication. The rest of the criteria includes how she is unaware of the switching until after the episode (Dissociative Amnesia), she is having trouble functioning again (distress), and it is not a normal part of her culture or religion. The patient does not meet determining criteria #5 due to the fact that the “switching” does occur in part from the effects of substance. The substance in question being Marijuana and occasionally Cocaine. E. Pharmacological and Non-pharmacological Treatment of Disorder One pharmacological treatment of dissociative identity disorder is Venlafaxine. Its brand name is Effexor XR, and Effexor. The usual dosage is as follows: For the immediate release tab, initially 37.5mg x2/day or 25mg x3/day, Maintenance 75-150mg/day,
