Case of Ivander Diagnosis F20.9 schizophrenia, specifiers; delusions, hallucinations Z62.820 Parent-child Relational problem Explain how you support the diagnosis by specifically identifying the criteria from the case study. The case study presented concerns about Ivander, a nineteen-year-old biracial male concerned about a change of behavior. Ivander reports that he has had a strange experience whereby he states that he went into another world and heard people talking; however, he could not talk back. He also stated that he could not trust his roommate because he thought he was taking his food and hiding it. Ivander reports that he hears voices outside his window in the dorm and fears trusting his roommate, whereby the roommate would put on television shows which refer to how Ivander was doing his classes. The aspect of hearing people represents auditory hallucinations which are among the specifiers of schizophrenia. During the mental status exam, Ivana reported having bizarre delusions and auditory hallucinations during those 3 weeks. Ivander also presents negative symptoms, for example, affective flattening, whereby he expresses little emotion in that his mood was anxious and concerned during the mental status exam. The symptoms presented by Ivander match the diagnostic criteria for schizophrenia. Describe in detail how the client's symptoms match up with the specific diagnostic criteria for the disorder (or all the disorders) you finally selected for the client. You do not need to repeat the diagnostic code in the explanation. In the case study provided, Ivander is concerned about his behavior change three weeks after the winter break. He reports that he has heard voices outside his window in the dorm, an angel's voice
calling his name. Also, in the mental status exam, Ivander reported having bizarre delusions and auditory hallucinations during the three weeks. The findings of the patient's mental status and history indicate that the patient has schizophrenia. Schizophrenia is a health issue that affects almost 1% of the population and has social and economic implications (Lin & Lane, 2019) . According to the American Psychiatric Association, the diagnostic criteria for schizophrenia entails two or more of the following, with each present for a significant portion of time during 1 month or less of being successfully treated. At least one of the symptoms must be delusions, hallucinations, or disorganized speech. Other symptoms include catatonic behavior and negative symptoms such as avolition, alogia, anhedonia, attentional impairment, asociality, and affective flattening. The mental status shows that Ivander presents with bizarre delusions and auditory hallucinations and hence meets the diagnostic criteria of schizophrenia. He also presents with affective flattening, whereby he expresses little emotions. According to Rahman and Lauriello (2016), people with schizophrenia experience a break from reality whereby they tend to see things or hear voices and hold on to false beliefs. Identify the differential diagnosis you considered. The differential diagnoses I considered include brief psychotic disorder, delusional disorder, and schizophrenia. I considered Brief psychotic disorder because Ivander presented with delusions, hallucinations, and affective flattening. The diagnostic criteria for brief psychotic disorder are the presence of one or more of the following symptoms, and at least one must be either delusions, hallucinations, disorganized speech, or catatonic behavior. Criteria B is that the duration of the disturbance episode is at least one day but less than one month with eventual full return to their premorbid level of functioning.
