This week's content examines and discusses strategies and standards of case management, the ecological perspective, and social pedagogy. We continue looking at Karen Campbell's case, as described in the Campbell Family Case Study: Karen Campbell, who was referred by her physician for counseling for case management services, reports being overwhelmed and depressed due to her children's recent legal and addiction issues. She reports being very worried about her children and consumed by their issues. As a result, her marriage is starting to suffer. Karen reports she has had anxiety about her marriage, as her parents divorced after her brother committed suicide as a teenager. She states her "marriage cannot survive ongoing crisis with teenagers" and she is "afraid that I am going to shut down and my husband will find attention somewhere else, just like my parents did." She reports not eating or sleeping well. In addition, Karen denies that her drinking is a problem but her family reports that she drinks so much on a daily basis that she often passes out at night. In your initial post for this discussion, respond to the following: 1. Identify the micro, mezzo, and macro perspectives of the therapeutic issues in this case and the reasons for those perspectives. Describe your assessment from an ecological perspective and a social pedagogy perspective. 2. Discuss the evidence-based interventions you would use to intervene for each area of concern. Use theories discussed thus far in this course to support your assessment and interventions. As stated by Jack (2012), within the ecological perspective, developmental outcomes are understood to be dependent on the interplay between strengths and vulnerabilities in the various settings within which people live their lives, including their families, friendship networks, school or work situations, neighborhoods, and the wider communities of interest and the society to which they belong. From an Ecological Perspective, the microsystems within the case of Karen and her family can be identified by her current alcohol addiction, her role within her family, her past relationship with her parents, and her interpersonal skills. From a mezzo system perspective would be that of her neighborhood and doesn’t really interact with others outside of her home except going to appointments with her providers. From a macrosystem perspective would be her culture as she is of Hispanic and Native American. Social pedagogy is a set of activities combining social aims with educational means to strengthen individual/community/social relationships (Hamalainen, 2015). Those suffering from depression have an increased likelihood of substance abuse while those with a substance use disorder are at an increased risk for depression. It tends to be a vicious cycle. Most people with a depressive
disorder who abuse substances do so to self-medicate. On a micro system level, I would recommend using cognitive behavioral therapy in individual sessions including cognitive restructuring, thought journaling, and mindful meditation. These techniques would be used together to show the connections between thoughts, emotions, and behaviors. I would also recommend structural family therapy. According to Tadros and Finney (2018), Structural family therapy was developed to analyze family relationships, behaviors, and patterns as they are presented within a session to evaluate the structure of the family. Structural family therapy utilizes activities such as role play to allow a therapy to examine subsystems within the family structure within the subsystems of parents and siblings. Structural family therapy allows a therapy to challenge a symptom, challenge a family’s structure, and challenge a family’s reality. Structural family therapy allows the therapy to identify the problem and evaluate the boundaries, hierarchies, role of symptoms in a family, subsystems, cross-generational associations, developmental practices, and a family’s life cycle. This strategy helps to decrease the dysfunctionality of relationships within he family system by restructuring the family system and implementing more positive pattens for the future. On a mezzo system level, I would recommend Karen attend support groups including AA or depression support groups. On a macro-level, I would recommend that Karen learn more about her culture.
