1 Conduct a Family Assessment Angel Boyce Department of Psychology – Northcentral University MFT5301: Systemic Evaluation and Case Management Dr. Byron Coley December 10, 2023
2 Conduct a Family Assessment Clinical Assessment Report Client #: TD01 Name: Tony Birth date: 6/12/1978 Age: 45 Date of Assessment: 12/10/23 Date of Report: 12/10/23 Assessment Administered: Genogram Administered by: Angel Boyce Report Written by: Angel Boyce Results : The genogram does not entail the process of assigning scores; instead, it is employed to discern patterns and collect information about Tony’s family history. Rationale for Assessment: Tony presented a disorderly household. The reason is to see hereditary patterns of substance abuse and other traumatic experiences within Tony’s family which may be the factors for Tony’s prevalence in alcohol usage. Background Information: Tony, a 45-year-old male, married to Dora, a 40-year-old, both African American. They both have 3 children: a 21-year-old daughter, a 23-year-old son, and a 25-year-old son. The 21- year-old cohabitates with her boyfriend, who is also 21 years old, and their 2-year-old son. Tony is a music producer and Dora works alongside him in his business. They both put all their energy into the business, which makes for a disorderly household. Observations: Tony presented his family problem and quietly discussed the familial patterns from his history. Explained with sadness how his parents' alcohol abuse disrupted the family and he had to be raised by his aunts and uncles. He regretfully mentioned that he too had problems with alcohol and wants help to stop drinking. Discussion of Results: There is a history on Tony’s side of the family of substance abuse and alcoholism, in addition to divorces and traumatic experiences. The prevalence of alcohol is great on his mother's and father's side of the family. It became worse after the death of his paternal grandparents. They were the major hierarchy for the entire family, and they were highly looked upon. After their deaths, and because of the substance abuse from his parents, the responsibility of taking care
3 of him was largely delegated to the aunts and uncles. Tony’s family lacked a hierarchical structure. A prevailing issue of dependency on substances was evident in Tony’s family. This transfers additional patterns of substance abuse onto his children and the lack of a hierarchical structure in his household. Clinical Implications and Recommendations: Through the analysis of these findings, I can make an approximate determination of when an issue first arises with the marriage. Tony followed the behavior of his parents, which could be the stress of the business, not necessarily the death of a family member. The confluence of familial demises in Tony’s lineage coincides with the commencement of substance misuse, implying a potential correlation and concern. Angel Boyce 12/10/23 Therapist Date
4 Clinical Assessment Report Client #: TD01 Name: Tony & Dora Birth date: 6/12/1978; 8/18/1983 Age: 45 & 40 Date of Assessment: 12/10/23 Date of Report: 12/10/23 Assessment Administered: Family Adaptability and Cohesion Evaluation Scales (FACES III) Administered by: Angel Boyce Report Written by: Angel Boyce Results : Tony; a score of 125 and Dora; a score of 121. Rationale for Assessment: I can utilize this assessment to place Tony’s family on a conceptual map, assessing whether an excessive level of adaptability results in unclear communication. Understanding the level of cohesion might elucidate issues occurring within the marriage or their family. Background Information: Tony’s alcohol misuse brings conflict into the family and a lack of structure. The children maintain a hierarchical structure over the parents. More focus is put into the business than on his family. He holds provision as the number one importance in the household. Observations: Tony and Dora agreed to take the assessment. Tony was less relaxed than Dora, rubbing his head at some of the questions being asked. He muttered a few words from his mouth while taking deep breaths in between. Dora was more relaxed while pondering over each question. Discussion of Results: Both achieved nearly identical scores, resulting in a state of intermediate flexibility and connectivity. They exhibited adaptability concerns for their relationship and their parenting roles. There was a slight difference between the cohesiveness and structure. But, it can account for the communication breakdown, and it can also explain attentive parenting and well- behaved kids. Clinical Implications and Recommendations: The situation impacts their relationship, but it does not impede their ability to parent. One
