She drove herself to the examiner’s office. She has been teaching for the past ten years and is currently attempting to transition into counseling. She has been reported to the school district's human resource office for being very difficult to work with and is often anxious and forgetful. She is being assessed because the school is considering hiring her as a school counselor since she has successfully completed licensure. The assessment is needed to determine her stability and ability to handle the new responsibilities. 2
HISTORY: This history was provided by Tina. Family History Tina is a single mom. She lives with her 5-year-old son. She reported that her son's father was abusive, so she has had to get a restraining order against him. He is no longer a part of her or his son's life. Tina was raised by both parents who are still alive and living as a married couple. She was born at Camp Lejeune while her father was deployed in Germany. She did not meet him until she was four years old. Tina and her family moved eight times to different military sites when she was between the ages of 6 and 18. She met her son's father in college at 19 years old and moved in together after graduation and had a baby shortly after. The child's father was removed from the home before Tina's son was two years old. Educational and Vocational History Tina speaks English and Spanish fluently. She has a Bachelor’s Degree in Elementary Education and a Master's in school Counselling. She indicated that her current job is the first one that she has had since leaving college, and she has been with this school for four years. During college, Tina worked part-time at a department store and also as a cashier at a fast-food restaurant. Medical and Mental Health History Tina reported that she was diagnosed with ADHD at the age of 11 years old, but her mother was opposed to her taking medication. Her mother gave her fidgets to take to school and reduced her intake of sugar. She has never followed up with her ADHD diagnosis. Upon becoming pregnant with her son, Trina took prenatal vitamins. 3
