Family Health Assessment for the Nursing Process Michelle Garcia Grand Canyon University: NRS-429V Family-Centered Health Promotion Terese Verklan September 28, 2014 Family Health Assessment for the Nursing Process Family health as a whole is an integral part of the development of healthy individuals. In order for the family to produce healthy individuals they need to function well holistically in all aspects of family life including health and wellness. If even one individual in the family is experiencing health issues which are not being addressed it can have a detrimental effect on the whole family. Therefore, it’s imperative that when performing an assessment on a family the nurse has as complete and thorough …show more content…
She states that they don’t formally exercise but have activities that are fun and active. Cognitive Perceptions The mother states that her oldest daughter has ADHD and dyslexia. Because of this she maintains the routine schedule for afterschool homework where she can be available to help and to encourage that they stay on task. She encourages learning by taking the girls to outings to the library and bookstore and has instilled a love for reading since an early age in both girls. She leads this by example being an avid reader herself. The mother feels that education is a team effort between the teachers and parents and is in close contact with her children’s teachers in order to accomplish this. Sensory Perceptions The mother said that they are non-practicing Catholics. The family does not attend church regularly. She teaches her children about God and the saints, right from wrong, celebrates religious holidays such as Easter and Christmas, and attends church on the occasional holiday. They do not follow any special diet restrictions that are preached by the church. Self Perception The mother feels that others see her as a strong independent woman. Even though she is independent she perceives herself as not being as strong as people think. She faces daily challenges and conquers most of her fears but she definitely doesn’t feel as strong as people believe she is. Role and Relationship Perception This family is not a very traditional
According to Stanhope and Lancaster (2008) “Each family is an unexplained mystery, unique in the ways it meets the needs of its members and society” (p. 550). Family nursing is a special field that involves the nurse and family working together to achieve progress for the family and its members in adjusting to transitions and responding to health and illness. The Friedman Family Assessment Model serves as a guide in family nursing to identify the developmental stage of the family, environmental data, family structure, composition, and functions as well as how the family manages stress and their coping mechanisms. From this data, three nursing diagnoses with interventions are developed.
Family Assessment The family chosen for this assignment are family friends who agreed to conduct the interview and discuss any health issues they had. Because some of the questions were fairly invasive into their private lives, no names are given. There are five members in the family, and they all participated in the interview. Questions are based on the eleven functional health patterns.
Although I enjoy all facets of nursing, I truly believe that it is in the field of family practice that the foundation of the nursing process begins. I have seen many patients over the course of my career without a stable, primary care healthcare provider. Health and wellness begins with day-to-day habits, and a nurse in family care is uniquely poised to give wellness advice and guidance. Family nurse practitioners often perform similar functions to physicians: making diagnoses; prescribing tests and medications; and helping patients over the course of their life trajectories lead more fulfilling lives. Primary caregiving is truly wellness promotion. It involves not simply healing the sick, but also reducing the likelihood that patients will become ill. I have seen so many patients with preventable illnesses that could have
A family health assessment is a process by which a nurse evaluates and describes the health status of a given family. It is a framework that helps to identify areas of potential risk for illness, opportunities for health education and actions needed to address these (World Health Organization, 2001). Specifics covered in a nurse led family assessment will include family history, perceptions about health, reports, health records, and any clinic test results. The nurse conducts an interview, compiles data and performs an appropriate
Family health assessment is an integral part of the formula used in creating a customized plan of care for the families’ health care. Family health assessment is also a tool that can be used to identify and evaluate the family’s health concerns, their life style and also helping families make good decisions regarding their family’s health. Family’s perception towards health and health promotion could be very different and unique, which makes the Family health assessment even more challenging for the nurses and health care professionals. Nurse’s have a moral obligation towards the society to help them promote their families health. The

A comprehensive family assessment provides a foundation to promote family health (Edelman & Mandle, 2011). 1987 Marjorie Gordon purposed 11 functional health patterns to use for guidance in order to facilitate nurses to have a frame work for the family assessment in. Gordon’s 11 functional health patterns help organizes basic family assessment information. This standardized format will guide nurses to complete the family assessment using system approaches, which will identify a patient’s
As the society we live in continues to transform, nurses need a comprehensive tool to assess family’s health patterns. Family units are influenced by environment, biological, mental, social and spiritual factors. Assessing these areas for health promotion and disease prevention will take all of these influences into consideration (Edelman & Mandle, 2010). A priority to a family focused health assessment approach is to adequately question, assess and examine these areas. A tool to assist a family health assessment using a family focused approach is Gordon’s 11 Functional Patterns. This paper will use these 11

Using Gordon’s functional health patterns to assess a family will guide the nurse in developing a comprehensive nursing assessment that is holistic in nature. Gordon’s functional health patterns are founded on 11 principles that are incorporated within the nursing practice. These 11 principles serve as a framework for a thorough nursing assessment in which to build a holistic and individual family care plan (Grand Canyon University, 2011). The author has developed family-focused questions for each of the 11 principles and utilized these questions as a tool to assess her own family. This paper will summarize the family assessment that was performed by the author and discuss two wellness nursing diagnoses that
The nursing intervention strategy that I will use to promote health for my friend’s family is the use of ritual/routines and family time. The easiest ritual a family can participate in, is eating together at a table for mealtimes. Routines build rituals in which rituals build a stronger family bond (Marquenie, Rodger, Mangohig, & Cronin, 2011). Families sitting and eating meals together are most likely to eat a well-balanced meal (Kaakinen, Gedaly-Duff, Coehlo, & Harman Hanson, 2010). Regular family shared mealtimes, helps children to feel like they belong and can build a trust. This also gives the family a feeling of being part of a group, allowing the sense of comradery (Kaakinen, Gedaly-Duff,
Gordon’s Functional Health Patterns is a system that was created by Marjorie Gordon in 1987. This system served as a guide to establishing a broader nursing database. These assessments include things such as evaluations of patterns within a family with corresponding details in an interdependent pattern. Gordon’s 11 functional health patterns consist of values/health perception, nutrition, sleep/rest, elimination, activity/exercise, cognitive, self perception, role relationship, sexuality, and coping. Each method is designed to collect information using a systematic and standardized approach. The collected information helps a nurse determine
America having been a very welcoming nation with welcoming people continues to experience immigrants from other parts of the world in search of better living standards and employment opportunities, hence has a high growing population with diverse cultures .This writer is a new immigrant from Africa .For that reason it is important for nurses to learn the health practices from different group of people to familiarize themselves with differing cultures. This writer did a health heritage assessment on two other different families .This heritage assessment tool was very helpful to this writer in getting

The family is the basic unit of society and a healthy family includes various traits, such as effective communication and listening skills, respect, trust and support for each other, a sense of humor and play, and balanced interaction between members (Kaakinen, Coehlo, Steele, Tabacco, & Hanson, 2015). In this discussion, I will consider a family I have encountered in my nursing practice, discuss what traits of a healthy family they exhibited, and review what roles I used as a family nurse caring for the patient and family.
