Health care management ensures that expertise professionals are serving clients to address issues that are arising. The manager’s role is to certify things are running smoothly in a health care organization. I decided to interview a nurse practitioner because that has always been my dream occupation, but I realized looking into the psychiatric part. I interviewed a licensed psychiatric nurse practitioner who works at Santa Clara Valley Medical Center in San Jose, Ca. According to the American Association of Nurse Practitioners (2016), “Autonomously and in collaboration with health care professionals and other individuals, NPs provide a full range of primary, acute and specialty health care services, including: Ordering, performing and …show more content…
What I learned prior to this interview through research was that medical records play a big role in a medical facility. There are chart rooms dedicated to these medical charts because they contain private confidential information regarding adults. HIPAA stand for the health insurance portability and accountability act of 1996. HIPAA electronic medical records have to strictly follow guidelines that have procedures in disclosing patient health information. Psychotherapy notes do not include: “medication prescription and monitoring, counseling session start and stop times, modalities and frequencies of treatment, results of clinical tests, and any summary of diagnosis, functional status, treatment plans, symptoms, prognosis or progress” (Holloway 2003). Also, the patient has to give consent in allowing the medical records to be provided to other medical providers to view. For the psychotherapy notes, “You do not have the right to access a provider’s psychotherapy notes. Psychotherapy notes are notes that a mental health professional takes during a conversation with a patient. They are kept separate from the patient’s medical and billing records. HIPAA also does not allow the provider to make most disclosures about psychotherapy notes about you without your authorization” (Office of Civil Rights 2012). Since I started school off as a nursing major at San Jose State University and have volunteered at many medical facilities, I

I later enrolled in a practical nursing program, in pursuit of my dream of becoming a nurse. While taking my prerequisites, I decided to volunteer at a local hospital in the emergency department. At the hospital, I assisted with transporting patients within the facility, preparing rooms for incoming
HIPAA protects the information of each patient and requires strict confidentiality of the professional that work in a healthcare institution, medical office and anywhere where healthcare services are provided. “HIPAA considerations include the need to be able to provide the client, upon request, with a log of caregivers who have accessed his or her chart” (Hebda & Czar, 2013, p.288). In this case I consider that Kevin shouldn’t access his mother’s record unless he is authorized by her or the nurse in charge of her care that authorizes him to have access to it to write notes, orders and document each encounter with the patient. The clerks unit can access the patient chart if they are required to give information to authorize a procedure, study or a copy for study results or for labs, otherwise they either can’t access them. In Kaneesha is not right either for Kevin to be
Through the many trips that my family has taken to the hospital due to various reasons, I have become fond of the hospital and its many wonders. After all the things that the medical field has done for me and my family, I decided to contribute to my local hospital to volunteer and help out those who pay visits to the hospital. Through my exposure to various clinical populations, I want to improve my ability to serve others in this capacity that can guide me towards the medical profession in the future.
I am currently a second semester nursing student at The Valley Foundation School of Nursing at San Jose State University. I will be begin my clinical training at El Camino Hospital this upcoming spring semester.
The website informs that under HIPAA, a patient has the right to obtain a copy of their medical records. An exception is doctor’s notes for a psychotherapy patient, but the patient still has the right to ask. Also, the individual has the right to submit a written correction to be kept with the medical records. Another important right is to have the medical records be kept private.
The HIPAA law dictates how our organization not only handles patient privacy and record keeping, but also how we handle patients’ request to view medical records for a variety of reasons. A patient in our organization requested to attain complete access to his records that include psychiatric records as well. Although the HIPAA law grants access to patients, there may be some crucial provisions that require special attention. I have done extensive research to provide our organization with the lawful protocols we must follow in order to protect our organization and our patient’s rights as well. HIPAA’s concise guidelines are what provide our organization and patient a clear stance on our responsibilities as well as limitations in the release
My passion is in psychiatric nursing. I am very much interested providing psychiatric care to the individuals suffering from chronic and persistent mental illness. Being equipped with over seven years of experience providing intense case management to this population, a non-RN position, I have finally found myself here. I attained my Registered Nursing (RN) license through the Entry Level Masters (ELM) program here at APU. My goal is to add needed competencies and skills to my existing experience as a case manager to be a successful Psychiatric Mental Health Nurse Practitioner (PMHNP). I find that my newly acquired nursing skills as well as case management proficiencies complement each other very well and are crucial to my future practice as a PMHNP. PMHNP provides care to individuals at risk for and suffering from psychiatric or mental health problems, taking the lifespan approach (from birth to death) (Thomas, 2013).
Daunting for the psychiatric geriatric Advanced Practice Registered Nurse (APRN) are a number of tasks, not dissimilar to the trials of Hercules. Registered nurses (RNs) transitioning to the APRN role, according to Kimberly Buzzelli, Psychiatric Mental Health Nurse Practitioner (PMHNP), have to develop the prior knowledge and willpower to separate from their RN peers and work collaboratively with Medical Doctors and system administrators who may be harried, busy, or self-absorbed in a number of other problems and pathologies. The need for the ability to diagnose conditions of health and illness and to prescribe treatments carries the Nurse Practitioner (NP) beyond their prior training and experiences, whatever the assumption
Let’s begin with nurse practitioners. What is a nurse practitioner? “Nurse practitioner’s (NPs) are advanced practice registered nurses who provide care to patients throughout the lifespan, from premature newborns to the elderly” (Nurse Practitioner, “2015). What is the basic job description of an NP? “They perform comprehensive and focused physical examinations; diagnose and treat common acute illnesses and injuries; provide immunizations; manage high blood pressure, diabetes and depression” (Nurse Practitioner, “2015). Upon interviewing Ms. Seal, a nurse practitioner at the Community Healthcare Center (CHC), she has kindly provided me with some detailed information on her description and duties, in addition to some vital information on the facility in which she holds employment.
The confidentiality for Health Care records is extremely important and it has a higher standard when comparing to other health care facilities. As far as Health records we have to observe very close to the HIPAA laws and be real cautious to do what it says. Additionally some psychotherapy’s notes are never released unless that is for continuation of care or if I receive a court order sign by a judge.
In addition, I come from a disadvantaged area with a shortage of physicians. I know from experience the difficulty in having healthcare access from these areas. I have volunteered in nursing homes and free clinics in these areas and I thoroughly have felt very satisfied and accomplished during my time there.
In my present role as a registered nurse at West Oaks Hospital, I am tasked with evaluating the psychiatric and medical conditions of up to 16 patients daily, and developing and directing a rotational system to manage the care of patients and coordinate workforce management. During this time, I’ve been recognized as a dedicated, ambitious, and reliable person who can

Service excellence is the ability of the service provider to meet or exceed the expectations of the customer they are providing the service to. Service excellence is important to all different fields where services are provided. In the health care field in the past few years, service excellence has become increasingly important.
