On 11/22/16 I was in store 14232 with APM Mary Cuestas conducting an overnight check. At 1:11am we arrived at the store and the doors were locked. An employee who was working putting merchandise away in front of the store, yell at us saying that the store was closed and turned around. I proceeded to knock on the door and it was not until I got his attention and showed him my ID that the employee opened the door. I asked the employee why the store was close and he responded “because they have to pack warehouse”. I asked SFL David the following questions: why the store was closed? He responded, “Because I am short employees and we need to work in warehouse” Does the SM know? He responded, “No” Who give you the authorization to close the store?
On 4/27/2018, I was conducting a business check of 8701 US 19 (Walmart) at which time I was approached by business loss prevention employee, Steven Uliano. Steven advised that a male subject identified as being Michael King was at a self-check out register and previously trespassed from this location.
On 11/4/16, I, Account Manager Enmanuel Cabrera, was notified via email by Shift Supervisor Massiel Perez that Security Officer Bailey Forte no called no showed. S/O Forte is usually off on Fridays, but was scheduled in advance, 0200-0600, to cover the open post. S/S Perez made several attempts to call the officers from 1st shift to cover the open post, but was unsuccessful to get someone to come in. Also on 11/3, during the afternoon parking lot detail AVP1 was 1 flagger short, all vital positions where filled with the exception of the parking lot supervisor/rover. As well as on 11/4, morning parking lot detail was short 1 flagger. Daniel Canela no call no show for the AM parking lot detail. All vital positions in the parking lot were filled for the exception of the supervisor/rover position.
On August 5, 2016 at 6:20 am I held Aaron Mills after the morning check in to discuss a situation that arose at Annandale High School. Present at the meeting was Craig Carney, Liaison and Steve Passino, HVAC Supervisor. The previous day Aaron was responding to a service call for the Athletic Directors office area Rooftop B1. Upon arrival at the jobsite I twice questioned Aaron if he had thoroughly checked out the Rooftop unit. Aaron responded “YES”. I then asked if he had checked the Freon charges in the system and Aaron responded “NO” as this is critical to a units operation. On 2/17/16 at approximately 8:30 P.M, Shift Supervisor (S/S) Enmanuel Cabrera was notified by Safety Specialist Matthew Marsicano that IH services associate Elizabeth Andrisoqie was asked to be removed by IH services Project Manager (P/M) Courtney Berger via cell phone. Upon arrival, S/S Cabrera spoke with IH services associate Elizabeth Andrisoqie and advised her that IHS P/M Berger asked for her removal. At this time, IH services associate Elizabeth Andrisoqie refused to leave. After, S/S Cabrera advised IH services associate Elizabeth Andrisoqie, that her refusal to leave the premises forces security to contact the State Police for trespassing. Then at 8:35 P.M, S/S Cabrera called GSCC and asked to speak with the On Call Loss Prevention
On 9/20 while doing the Tardy Sweep in the Cafeteria, Joseph Santos came into the Tardy Zone area (Cafeteria). I heard Officer Lee, say “Hey give me those cards.” Joseph refused to give Officer Lee his cards and said you are not getting my cards and put them in this book bag. I asked Joseph to give up the cards and to not make this bigger than it has to be issue. Joseph eventually gave up cards after being prompted to several times.
Debra McCann and two of her children (Jillian, and Jonathan) were shopping in Bangor, Maine Wal-Mart on December 11th, 1996. After about an hour and a half, the McCann’s paid for their purchases and proceeded to leave the store. On the way out two Wal-Mart employees (Jean Taylor and Karla Hughes) blocked their path to the exit and stood in front of the McCann’s’ shopping cart. Note Taylor may have actually put her hand on the cart. The employees told McCann that her child had previously stolen from the store and was not allowed in the store. Upon my arrival I contacted the complainant Nicholas Swanson and he told me the following. Around 2000 hours a white female, later identified as Sonnie R. Youngblood, entered the store through the front doors. Youngblood stopped at the popcorn stand in front of the store and picked up three bags of popcorn. Youngblood then walked to the west part of the store and returned to the front of the store with no items in her hand. Swanson said was notified by his employee that Youngblood might have placed items in her bag. Swanson checked the video footage and saw Youngblood place several items

Upon my arrival, I made contact with the complainant/manager, Jennifer Lima. Lima advised an unknown suspect stole 4 items, 20 minutes prior to my arrival.
I, Enmanuel Cabrera, was asked by Integrity Manager LoraLee Sposato to do some research on Integrity Associate Tiphanie Ortiz because Loralee was informed that Tiphanie was off task on 3/14/16. Upon review of lenel’s and the video surveillance system, I was able to determine that I/A Ortiz entered the building thought turnstile 6 at 9:06:26 P.M. Afterwards at 9:12 P.M, I/A Ortiz went to the Light duty table where she remain until lunchbreak at 11:20 P.M. I/A Ortiz did get up at times, to occasionally go over to an outbound sort associate work station to converse but for the most she was at the Light duty table behind the H.R desk. After lunch, I/A Ortiz left for the day at 12:46 P.M.
I responded to 212 S. Three Notch Street in reference to a trespass call. Upon arrival, I made contact with Ashli Jordan Baxley. Baxley stated that Christopher Ryles came in the store on this date, and dropped a bag full of his personal items. Baxley stated that Ryles was then in and out the store. Baxley also stated that Ryles didn’t purchase anything the entire day. Upon officers arrival, Ryles was at the cash register buying candy. Baxley advised to Ryles that was banned from the store indefiantly.
Little did I know the lady latter on called the coporate office and made all sought ofounded allegations against me. Luckily for me, my manager was there on that day, so she knew what really happened. She told the district manager that most of what the lady said were untrue. Thanks to her, I was able to keep my job. Some weeks latter, I checked out that same lady, and she had bought a lot and had three carts.
Upon arrival, I met with the Walgreen's Lost Prevention agent Robinson, Dartanyon DOB 10/30/64, who currently had two (2) female subjects detained in the rear of the store. The females were identified as Matthews, Latonya M. DOB 09/07/70 and Harris, Maria A. DOB 11/20/77. They were both checked through LEADS, which revealed clear with no wants or warrants. Robinson related he observed Latonya and Maria on the floor place items inside a shopping cart. Dara Manilli Acanto is a Business Administration senior student major in Business Management in West Visayas University in the Philippines. She is graduating on March 2016 with honors. Despite the hectic schedule at the university, Acanto manages to work during the night, and go to school during the day. Acanto works as a full-time call center agent for Teleflora company, headquartered in Los Angeles, California for processing floral orders to customers throughout the United States and internationally.
