Joivita Bradford was a member of the 2014 Public Health Associate Program, where she was assigned to work at San Joaquin County Public Health Services (SJCPHS). Located in the Central Valley of California, Ms. Bradford established the SJCPHS Safe Routes to School Network and conducted walking environment assessments near local elementary schools. The findings were used to guide schools in the creation of handbook policies, alternative walking routes for students, Geographic Information System (GIS) maps, and transportation plans including the City’s Bicycle Master Plan and Safe Routes to School Infrastructure Prioritization Plan. An “audit failure” is a situation in which a professional auditor fails to detect a material error in the financial statements of the company they are auditing. The audit failure in the situation of Rita Crundwell the failure was exacerbated by the fact that the auditors continually signed off on the misstated statements for years. Crundwell is responsible for many of the deficiencies mentioned, such as the missing funds and the incorrect invoices. However, she is not the sole person responsible for this fraud. Bea was in need of additional income, she wanted to find a female to rent her spare room to. The social worker had a friend, “a woman who might be interested” renting the room (Hoffman, & St. Clair, 2003, p. 46). On the morning of May 7th, 2000 the murder of Mary Ann Stephens occurred. She was a tourist to Jacksonville, Florida along with her husband. The two were staying at the Ramada Inn Hotel and had just came from breakfast when they were approached by a young black man who held them at gunpoint, took Mrs. Stephens' purse then proceeded to shoot her between the eyes. When the police first arrived, the elderly woman's body was several inches away from the path on which her and her husband were walking along to get back to their room. Julie Gurnick is a broadcasting major at Point Park University from Jeannette, commuting an hour to campus each day. She was interested in Point Park University because it offered a good program in communications. Gurnick also holds an associates degree in business communications form Westmoreland County Community College. After graduating from Point Park University Gurnick aspires to become a sports caster. Gurnick is currently a free-lance employee for the Tribune Review as well as a dietary aid at St. Anne Home.
Latasha Willis, an intern at River Valley Area Agency on Aging, is currently at Troy University to obtain a Masters in Social Work (MSW).Latasha will be interning with the agency until July 2016 which is when she graduates with her Masters degree. She has earned her undergrad in Social Work (BSW) as well from Troy University in 2014. After graduating with her BSW, she worked with the Division of Family and Children Services as a CPS investigator in Butler, Ga. She also worked at the Division of Child Support Services as a Child Support Agent 2 in Columbus, Ga. She plans to continue working with seniors after her gained experience with interning at the agency.

Shadale Gillespie is a Direct Care Provider for our agency since February 2015. Work is not conistant due to the amount of Respite hours authorized by the AZ DDD department for the child she services. The authorization avaialble hours have ran out and she has not been able to work with the child since July 9, 2017. Last paycheck on July 25, 2017 for gross amount of $400.00. Bi-monthly pay periods, rate of pay $10.00/hour on average of 10 to 20 hours per week when we have authrizations available to deliver services.
Could you please assist me? My associate was reviewing appeal # APP-2018599 that was completed by Priscilla Wilkerson. She notice that all the claims checks were suppressed when they were not advised in the SOW to processed this way. In order for all 18 claims to be reprocessed, Danielle has been advised to redo the SOW. Since, this was not her error, I think that these claims need to be reprocessed again correctly by Priscilla or another claim analyst. Could you please review and advise? On April 8, 2016 at approximately 1500 hours, I made contact with Heather Curtis at 8307 Centennial Road in reference to a domestic dispute. Mrs. Curtis stated that her husband Marc Curtis sent her multiple text messages to her phone stating that she couldn’t have the kids and that the police would have to kill him before he goes to jail or give her the kids back. This was the result of an argument after Mrs. Curtis refused to pick him up from work because he had taken her EBT card. Mrs. Curtis stated that he hasn’t been acting normal and may possibly be going through a psychotic break. Mrs. Curtis was advised to go to the Prosecuting Attorney’s Office to get a restraining order and given a copy of this report number. Mrs. Curtis was also

Tamika is assigned in working with the family due to a report of Child Abuse.
