There can definitely be an issue with this patient being seen by several physicians. In this case, I would assume no one had the time to accurately collaborate on the best treatment for the client. The new physician the patient is seeing probably is missing out on essential information about the patient’s health history and background. Gui’s over-the-counter medications could be counteracting with his heart medications. I would recommend Gui needs go to necessary follow-ups, so his symptoms can be addressed and possible side effects can be monitored carefully. As a case manger, I would only speak with the son on these issues after my client had signed a release and consented to me discussing this information. As with most clients involving patients and building a collaborative relationship is of extreme importance, connecting with the client and building positive, professional relationships. As attachment is an important of all relationships, and a biological need for people, building a relationship with the client is important for the clients, so they can work in a trusting relationship. Allowing the client to see that as a practitioner you can work collaborate with goal development and then working towards achieve goals or adjusting them during the counselling treatment plan. This will not only be this beneficial for clients, it also supports the practitioner as they develop as a counsellor, reflecting on their own practices with clients.
Once the client’s needs have been established and the resources have been located the case manager must constantly review the progress of the client and determine if continuing case management service is needed. Terminating the relationship is never an easy process; the case manager must carefully create a termination process depending on the client’s level of dependence in him or her. If a client is heavily dependent on the support of the case manager, the need for a gradual decrease in the amount of contact with the client may need to occur before termination.
The case managers' role is diverse. It consists of screening, assessing, planning, implementing, following up, and evaluating the situation of the foster child assigned. They are essential in the child's case plan by acting as a liaison that connects the foster parent and the state. D-The patient arrived on time for her appointment and stated to this writer as they walked to the office that things has gotten better. According to the patient, she knows her triggers to her anger and is willing to address it before it worsen to the point of walking away from the negativity. The issues in her home whereas it's her sister and her son, also, her brother and his family living with her as well tends to give her issues such as disrespecting her home and space. The patient then informs this patient that her brother, who was serving 22 months for drugs was released, staying with her until his parole approves his apartment. The issue in the home is not her brother, but his signigificant other and his son not respecting her home as mentioned before including her sister and her son. This writer and the patient discussed alternatives addressing the anger and dealing with those not respecting her home. At first, the patient mentioned about going into a shelter, but realized she cannot leave her 72 year old mother in the house by herself because as the patient reports, people will take her mother for granted.
The case manager regulates use of resources to provide the most efficient care for their clients, which can possibly lead to the client being able to care for himself/herself. Another principle that one should be aware of is known as continuity of care. There are two parts to this principle, the first one states that from the beginning of time when a client calls the case manager/agency to the very end and beyond the ending of the care by providing a short-term line of assistance if necessary.
