To schedule an appointment please call the office or leave a message. I will return your call as soon as I can. Typical individual session with general run around 50 minutes. In some cases addition time may be necessary which would add another 40 minutes to the session.  This office is operated on appointments only. Should an emergency happen before our appointment please contact 911 or your local emergency services.

Step 1- Register Patients/ Check In - You can do this either when patient calls to set initial appointment and ask for their insurance information via phone to confirm prior to appointment. When patient comes in ask for insurance card for your records, along with any documentation that is needed. Example Referral or Prior Health Records.

The pt Poteat, Edward L is supposed to tx today at unit 1087. The unit 1087 is requesting the referral source to send a shortened and missed treatment report in order for the pt to be accepted. This report is not one of the requirements that is needed in order for a pt to transfer to a FKC unit. Also the referral source stated the she sent the report several times and received several conformations and clinic 1087 is stating that they never received it. The referral source and the unit 1087 have been in contact and I am just now finding out about this. Please let me know if the pt is accepted and to see what we can do regarding this patients

Dillan remained in the MHS G. Tate’s home from 07/01/2015 – 07/13/2015. The youth continued to display unhealthy behaviors of lying, stealing and allegedly smoking marijuana and alcohol. Dillan completed weekly independent living skills at the foster home. The youth required constant supervision. Dillan does not receive individual therapy, and no family visits established by DSS. The youth gave bio-family the address and telephone number of foster care placement.

The suggestion system is good to have. Hopewell Center has a very high turnover rate and loses a lot of doctors as well as staff. If a doctor resigns and you have a client that is schedule to see that doctor the only way you will know that you need to reschedule your client is when you and your client is at that appointment. Therefore I suggest to my supervisor in the team meeting to speak with the director of the clinic to inform us when a doctor will no longer be with Hopewell Center or will be present for the appointment. Sometimes we will be informed and sometimes we will not be informed.
