In August 2006, I received a promotion as a Medical Case Manager, for the Ryan White-Early Intervention Services Program at the Community Health Center. The Ryan White program helps individuals receive comprehensive primary HIV medical care and other support services. In my experience as a medical case manager, I am the first contact person when a client is scheduled to see a medical provider at the health center or seek other supportive services. As their medical case manager, my responsibility is to do the client’s first initial intake by getting the client’s demographics, medical coverage information, and link the client with healthcare services. An intake and assessment is where an initial meeting is conducted with a case manager and a client or client’s caregiver to gather information in order to address the client’s immediate needs. The purpose of an initial meeting is to make sure the agency is able to provide the services requested by the client or caregiver and the initial meeting may also be used to screen clients to determine if there is a need of case management services; and if so, the intake can determine the type of case management the client need. The case manager assess the client and helps by providing appropriate support and by making referrals to other agencies where help can be provided to help the client accomplish their treatment goals. An intake can initially start over the phone or face to face by gathering as much information as possible in regards to the client. An initial appointment should be schedule on the first point of contact with the client or caregiver. Many agencies are flexible in terms where the intake and assessment should occur. It should always be conducted in a place that is safe and convenient for the client or caregiver. The case manager should notify the client of any documentation needed from them in order to facilitate the intake process. The process of assessment involves both the client and case manager identifying the issues, strengths, and service/support requirements that will inform the planning process. A comprehensive analysis of the information gathered through interaction with the client will support a detailed assessment. Case planning is a multifaceted exercise that sets the scene for what follows between a client, the case manager and formal and informal supports. Case plans are client centered and may be developed over the course of several meetings with the client. When planning a termination strategy the case manager determine if the client will lose access to the services and resources once the relationship is terminated. While this may not always be the case often times case management services provided through community based waiver programs will be terminated. Clients come to the window and sign in on a sheet with the type of assistance they are requesting. We ask them for their photo identification card and look up on the computer to see if they have an existing file. If they have an existing file, then we retrieve it from the filing cabinet, if not, we create a new file for them. Once we have the existing file, we update any information such as dependents, what number of assistance they are receiving since they cannot exceed six, and if they have a change in address. All files are labeled with the first two letters of their last name. Once the files are updated, it goes to another worker who enters the information in the database and then to the caseworker. The caseworker meets with each client to find out why they need assistance and to document this in the database. Reasons could be due to unforeseen expenses such as medical or a car repair. After the caseworker has met with the client, the client then has their food order filled. Since February is a slow month, clients are usually are helped immediately and there is no long wait. Clients that are new must fill out a two page assessment form which is put in their file. After the clients have been served, their files are filed back alphabetically in the filing cabinets.
