On 10/28/16 at 1838 hrs, I was dispatched to an assist citizen at 5660 82nd Ter N Pinellas Park, FL 33781 regarding a juvenile hitting the complainant vehicle and leaving the scene. Upon arrival, I made contact with Stanley Wooten who advised a juvenile was riding his bicycle through the neighborhood and struck his vehicle. Wooten stated when the juvenile hit the vehicle he was worried for his well being because of the impact on the vehicle. When he questioned the juvenile, the subject started telling Wooten to “fuck off” and rode off with his friend’s away from the incident location. Wooten advised at this time he doesn’t want to press charges for the hit and run, he just wants me to check on his well-being. She advised her son Curtiss Kaiser just left 10 minutes prior to my arrival advising he hit someone’s car. Tripp stated Kaiser hid his bike in the back yard and walked on foot towards his friend’s house. Tripp called Kaiser and advised he needs to come home immediately. Kaiser arrived on scene and I made contact with him regarding him hitting Wooten truck. At approximately 1:28 a.m., I arrived at the scene and met with Trooper Otten, who advised what had transpired prior to my arrival. Trooper Otten stated a blue 2014 Ford F-150, driven by Eric A. Wenzel, was traveling northbound on Illinois Route 4 south of Norton Lane. While traveling northbound, the blue Ford veered to the left and entered the southbound lane of the roadway. Once in the southbound lane, the front passenger’s side of the blue Ford struck the front passenger’s side of a southbound silver 2003 Ford F-150, driven by Alan L. Schilling. Trooper Otten stated both Mr. Wenzel and Mr. Schilling sustained incapacitating injuries in the collision Trooper Otten advised the silver Ford’s front seat passenger, Sarah L. Wagner-Schilling, sustained life-threatening injuries in the collision and was not expected to survive. On Thursday, July 14, 2016, at approximately 0847 hours, Patrolman David Stamets had a vehicle stopped on the 100 block of East Oak Street. Immediately after Stamets stopped the vehicle Your Affiant positioned his vehicle on the 100 block of South White Street near the Oak Street intersection facing southbound. When exiting my vehicle your Affiant observed a gold Pontiac sedan accelerating northbound on South White Street at an unsafe speed towards Stamets and your Affiant's location.
On Sunday November 24, 2016 at approximately 16:02 hrs. Officer L Wilkerson and I Ofc. R. Holcomb were dispatched by dispatcher B Coleman to 304 Hopkins Drive in reference to a disturbance. Upon my arrival I made contact with Ms. Shawntavia Lucky (complainant) who stated that her son Mr. Jousha Lucky (victim) got into a physical altercation with two black males.
On October 4th 2016 Deputy Akins was dispatched to 8821 Centennial Road for an accident involving a red truck that left the roadway and struck a parked vehicle. Upon arrival Deputy Akins saw a 2013 Red Dodge 1500 in the ditch with the truck’s front end smashed into the rear end of a 1001 Honda Civic. In the driver seat of the dodge truck was a male Caucasian. Deputy Akins recognized the male as Evan Case. Deputy Akins asked Mr. Case if he was injured. Mr. Case replied with I can just pay the deductible and leave right. Deputy Akins asked Mr. Case again if he was ok or if he needed medical. Mr. Case replied no.

On 10/30/2016 at 0251 hours Ptlm. David Stamets and I were dispatched to the unit block of S. White St. for multiple males in the road fighting. We arrived on scene and I observed three males walking toward Main St. I instructed the males to stop, but they did not comply with my request. The males looked back and continued to walk away. I again gave verbal commands for them to stop, two of the three males stopped, while one continued to walk away. I walked to the two males who stopped while Stamets proceeded in the patrol car to make contact with the third male, who he caught up to.
On 03/08/2017 Deputies were dispatched to the area of 120th Ave and Oliver Road for a traffic complaint. The reporting party advised a vehicle was parked near the tracks and when a train approached three occupants got out of their vehicle and stood in the roadway. Other deputies were en route to this call when another call came out and other units were needed elsewhere. After finishing with my current assignment I responded to this call.
I spoke to the drivers who were all out of their vehicle and appeared that they were going to transfer information. I asked “Who was injured?” They replied that everyone was OK but one driver, later identified as Erin Feeney had injured her knee/leg. I requested Goodwill Ambulance to check on Feeney. I then asked “What happened?” Becky Boone who was operating a 2017 Silver Ford Focus stated that she believed a white vehicle struck her from behind causing her to collide with the vehicle in front of her.
On Thursday, August 31, 2017 at approximately 2232 hours, I was dispatched to a residence across the street from 2220 University St in reference of a suspicious person inside of a black truck. Upon arrival I spoke to the complainant Ms. Delisa Smothers. Ms. Smothers stated that at approximately 2230 hours, an unknown black male went inside her neighbor's truck located across the street at 2221 University street.
On Thursday, 05/18/2017, at about 9:27 p.m., partner Officer Griffin (#3135) and I were on uniformed patrol in a marked patrol vehicle (Unit #21) as 3A21. We were dispatched to an injury traffic collision at Lake Ave. and Maple St. Upon our arrival, we saw three vehicles that appeared to be involved in a traffic collision. While we were assisting with the scene, we saw Victim Captain Konrad struggling with the suspect, later identified as Kevin Hendrix. Officers Griffin, Riehl (#7454), Spatz (#8058) and I grabbed Suspect Hendrix and took him to the ground. I placed properly adjusted, double locked handcuffs on Hendrix. Officer Griffin and I placed Hendrix in our patrol vehicle.
On 05/23/2018 at approximately 2226 hours, I, Deputy A. Martinez Vazquez (117015) responded to 211 Mission Rd., Oviedo, Seminole County, Fl. In reference to a disturbance inside of the residence. While in route to the residence, I was notified that the disturbance turned physical against a mother and daughter. Before I arrived to the residence I was notified that the 15 year old female juvenile was inside of her room.
SSG Stoutenburg stated he had been zonked from P.T. and went home and took his Son to the bus stop. While at the bus stop he saw a vehicle approaching at a high rate of speed and stepped in to the road to get the vehicle to slow down. He stated as the vehicle came closer, that the vehicle was not complying with his wishes to get the vehicle to slow down. He felt that going towards the speeding vehicle was going to protect the 5 to 7 children who were waiting at the bus stop at this location.
