CASE STUDY
INTRODUCTION:
My case study is about Mrs JL, who is 81 years old originally from Ireland and migrated here in New Zealand 4 years ago. She was admitted at St. Andrews Village, located at 207 Riddell Rd., St Johns last 24 March 2013 as a private payer with a diagnosis of Alzheimer’s type of Dementia and Psychosis (Delusion of Persecution).
According to her daughter, she decided that her mother to be admitted on the said facility as she witnessed, she is neglecting herself in her previous place. She also stated that her delusions intensified at home, which put her in vulnerability.

Cognitive Skills for Decision Making: Her intellectual Skills have deteriorated in decision making her last Mini-Mental State Examination (MMSE) was 9/27 on 29 July 2016.
Memory/Recall Ability: She distinguishes caregivers though she does not call them by name. She knows the way going to activities room. She has a short term memory problem.
Periodic Disordered Thinking or Awareness: She appears to get impatient or side tracked at times when conversing with other residents or staff. Her mental functioning does not vary over the course of day.

Nursing care is a dynamic field of practice. The way it looks today is far out greater intense and very structured. It advances itself by the use of nursing theories and evidence based practice. Policies and procedures constantly change with the advancement of technology and science. While caring for the patient in the given case studies, a nurse involved utilizes practical knowledge, a culture care model and transpersonal caring relationship to attain a caring environment (Smith & Parker, 2015).
The aim of this study is to provide a detailed account of the nursing care for a patient who is experiencing a breakdown in health. One aspect of their care will be discussed in relation to the nursing process. The model used to provide an individualised programme of care will be discussed and critically analysed.

Example: Her mother said “do you love mommy” and Haper replied “yes” the mother asked “how much” and Harper replied “sixty dollars”.
Mrs X has diagnoses of early stage of dementia moved in to our community from her own home, where she was independent and on the assessment family did not mention any concerns.
CCIB LPA Perryman-French received a call form Delores. Her mother, Bernice Howard (DOB 02/24/1925) has Alzheimers and dementia. Her mother was living at another facility, when she required an awake night staff. She was moved into this location around June 20 or 21st, 2015. Delores left the area and then returned on 07/05/15. During that time, she had her son checking on her. Sometime around 07/01/15 or 07/02/15, her son told her that he was told there was an altercation between staff "La" and Bernice in the kitchen. The result was bruises on her arms (where she was grabbed) and a visible scratch. Bernice informed her that she was hit, slapped, grabbed and scratched and also reported that she responded by getting her (La) back. The family had
----- Clinic presents a black male 68 years old. Currently experiencing dyspnea and lethargy. For the past week he has been having a increase of difficulty breathing. Complains of alternating periods of sweating and chills. Other symptoms he has been experiencing is a productive cough with expectoration of thick yellow sputum. Patient is a ex- smoker, he was a 40 pack year history, denies smoking, stopped over 10 years ago. Medical history includes chronic bronchitis, hypertension, MI five years ago, has had a angioplasty, and denies chest pain since having angioplasty. Current medication combined albuterol/ipratropium MDI, nebulized albuterol prn, captopril, and hydrochlorothiazide.
R/s Mr. Joe Adams has some dementia issues and he is wheelchair-bound. R/s Mr. Adams’ daughter-in-law Georgia is his caregiver. R/s the home looks as if dumpsters are being dump there. R/s the family has a dog that uses the bathroom inside the home. R/s the home is infested with bugs. R/s there is old food with flies. R/s the smell of filth and dog will knock you off of your feet. R/s Georgia is giving Mr. Adams unprescribed Xanax to get his debt card, so she can buy groceries and pay bills. R/s the home is being foreclose and Mr. Adams will be homeless because Georgia said she will leave Mr. Adams there. R/s Mr. Adams son Jerry is also in the home, it is alleged Jerry has mental issues. R/s Mr. Adams wants to place in an assisted living setting.

The components, which would be challenging for Heather is, recalling of facts for computing and reading ability for word problems. The reason recalling of facts for computing would be difficult for Heather, is she has short-term memory weakness, which affects her ability to hold onto facts, consolidating the information to her long-term memory to later retrieve or produce the correct results. Therefore, she fails to elaborate on the incoming information activating her prior knowledge and relating it to be learned information. For example, if she was given this math problem, “Joyana filled her bucket with 5 pounds of shells, if a friend comes to add 15 pounds of shells, and another friend comes to add 17 pounds of shells, how many pounds does she have altogether?”. She would fail to retrieve the information automatically and fluently because she is unable to hold onto the details to solve the problem.
The answered each question without hesitation, while she listened intently to each question without requesting for any of the issues to be rehashed while maintaining eye contact. Her answers were short and concise, without extra verbiage; while she did not become, emotional or showed any passive or aggressive behavior.
