Communication is the foundation of exchanging information from one person to another, whether it is in the form of written, spoken or by body language, communicating is vital to everyone. Therefore, effective communication is imperative within the healthcare, for it is through communication relationships, information and understanding are obtained. The purpose of this paper is to read the critical thinking exercise with Nurse Oliver Witte and describe the problems that are presented, whose problems it is? What should Olivia do about it? In addition, I will discuss what mode of communication Olivia is using and in what order should she communicate with first, along with who else needs to be involved with this her communication. Lastly, what type of leadership and management strategies should Olivia use?
Problem Outlined
There are several problems presented in this exercise. One is Nurse Olivia is trying to complete several tasks herself. Secondly, the representatives from the dietary department expect the nurses integrate the protocols instead of doing it themselves. Thirdly, there is no collaboration between the interdisciplinary (doctor, home health, dietary, and nurse), and last, of all, the physicians are not cooperating thus causing conflict, and thus the communication between them is broken. For this paper, I will be discussing the problem of communication and the lack of delegation within the communication. Whether the problem lies with the interpersonal communication or the fear of lack of knowledge as to how Olivia can delegate or communicate effectively, the situation at hand is causing conflict and various power struggle within the disciplinary group (Huber, 2014).
One significant problem that must be addressed and resolved quickly is both the conflict and attitude of the doctor. It is well known that physicians’ has had complexes about themselves and have been put on a pedestal when it comes to hierarchy. They have always been hard to communicate to that they never appreciated nurses questioning their decisions. Today thankfully this is changing. Within a health care setting communication is a necessity. This communication not only includes the need for professional communication but also the way in which information is shared to the patient and to other healthcare workers. Another important aspect of health care worker such as a nurse is the effectiveness off a handover. Within the video, Effective Communication in nursing these three aspects of communication (Professional communication, provision of information and handover) were seen and will be analysed further, within this essay. These will be analysed through the three aspects, the care of the patient, the image of the individual nurse and the health outcomes of the patient. All of these three aspects of communication are vitally important to the overall patient needs.
Nurses have communication issues with physicians with leading the to errors that are occurring in the health care team. Nurse are feeling pressured into giving a medication that they have a question the safety and they feel they are and unable to effectively communicate their concerns. This leads to a poor outcome for the patients. Its is very important to ensure we have effective nurse-patient and interdisciplinary communication so we ensure our patients are being taken care of properly. Each health care team member plays an integral role in patient outcomes. The health care communicates their recommendations. No health care team member acts in isolation and the patient trust that her team is working as a whole to help her to achieve the best outcome. If the nurses and physicians don’t always communicate information in the same matter then a poor outcome occurs. Nurses look at the broad picture and want to give details, and physicians want the pertinent clinical information quickly. The nurses are taught to use SBAR reporting. The nurses briefly explain the situation with the patient, gives some background information about what led up to the development, provides an assessment and then makes a recommendation to the physician. By doing this, it helps to improve communication efficiency and accuracy. Communication plays a vital role in the healthcare setting, as the relationship with the healthcare professional sets the tone of the care experience and has a powerful impact on patient satisfaction. It is “the shared process in which messages are sent and received between two or more people which are made up of a sender, receiver, and message in a particular context” (cite, date). Mr. Comer was admitted to his local community hospital for respite care. He has suffered multiple, acute strokes in the past, which has left him with severe disabilities. These include paralysis rendering him immobile, aphasia (speech loss) and dysphagia (swallowing difficulties). He relies on carers for all normal activities required for daily living (Roper et al 1996) and is advised to have a pureed diet and thickened fluids.

How often does a registered nurse effectively communicates the medication and the rationale and or importance for taking the medication to their patients? I chose goal nine from the fifteen Patient Safety Goals created by the The Joint Commission on Accreditation and HealthCare Organizations. Goal nine, or NPSG.03.06.01, focuses on maintaining and communicating accurate patient medication information. Whether it is from the physician to the nurse or the nurse to the patient. Lack of proper communication often leads to medications errors. In fact, in 1999, the Institute of Medicine estimated approximately 7,000 deaths are caused by medication errors annually. Communication is a big role in the nursing environment. Being able to identify communication factors both verbal and non-verbal can help to see how messages are delivered and interpreted differently. It also helps to see if the non-verbal communications match the what is being said, as non-verbal communication is just as important as the verbal. This is also good practice for how you communicate with others around you and how well you can read others reactions to what is being said. Like if they understand what is being said or if they are just saying they understand when they don’t. an example of communication can be seen by the female nurse and the male police officer, the message that the female nurse was trying to give was not being interpreted
When conflict interferes with the nurse-physician relationship it can and will impact communication and eventually impact patient care. In the context of this paper, according to Tabak and Orit (2007), a conflict is defined as a situation arising where one party in a workplace relationship perceives that the behavior and objectives of the second party are not compatible with, and therefore threaten, their own. The background example suggests a conflict that has arisen in a nurse-physician relationship that has caused a nurse to feel uncomfortable and apprehensive toward a physician. Therefore, if Leslie and Dr. Eastman have to collaborate for a patient’s care their working relationship may be damaged and thus affect patient care. Tabak and Orit (2007) continued on by stating that the working relations between doctors and nurses, as any set of relations between two collectives in the workplace, are affected to some degree by interprofessional conflicts. There are many factors that can affect the relationship of nurses-physician relationship: gender differences, socioeconomic status, and gaps in education. In some instances, it is related the a physician’s having the “Good Old Boy” mentality, and believe that the nurse is nothing more than a servant to the physician. The conflictual factors (Tabak & Orit 2007) include the fact that nursing is still by and large a female profession while doctoring is still largely male, and the physicians higher prestige.
In the authors placement area she feels there is a lack of communication between different members of the professionals that have been mentioned in the essay. There seems to be no structure mechanism to be able to pass on information of patients, the opportunity to communicate with the GP is limited to casual encounters in the reception area of the practice. The definition of communication necessarily involves “an exchange of ideas and an ability to stimulate meaning for the recipient of the message”(Grover, 2005). Effective communication can be influenced by varies components such as gender, hierarchy, trust, self-disclosure, empathy, mutuality and context. This paper will discuss the importance for nurses to use effective communication skills in regards to different social groups and use specific interpersonal strategy. Additionally it identifies how nurses can enhance their communication skills to prevent potential communication barriers between a nurse and patient thus resolving problems or conflicts with effective problem-solving strategies (Davis, 2009).
