This week my goals were based on the critique of my SOAP Note HPI narrative by my professor, which her feedback gave me the opportunity to re-evaluate and focus on the organization of patient information and documentation findings. Patient documentation is crucial in a collaborative health professional setting, due to the written documentation must be clear and easy to use and understand by others. I’ve only been here for two weeks, yet I have learned so much. I plan to be progressive in my career and advance to higher levels in healthcare. Though I am aware that becoming a medical assistant isn’t going to happen overnight, I plan to work hard so I may achieve this goal. This healthcare career requires a lot of skill. Medical assistants have to be very competent in communication, however, I am not very proficient when it comes to communicating.
Medicine has the capability to cure or alleviated illness, and it even brings people together such as a doctor and the patients that seek their consult. My practicum class has helped me learn the skills necessary to become a physician, which includes taking vitals, patient assessment, cardiopulmonary resuscitation, etc. Likewise, my experience in the hospital setting has helped me achieve one quality of becoming a doctor, which is communication. I wrote up an assessment on a therapy session with a 74 year old female as she played Rummickub. This was the first documentation experience I had during my fieldwork. An example of my first write up was: “Pt initiated 30 min session in TR clinic working on cognition, UE strength, fine motor skills, scanning, sequencing, math skills, social skills, activity tolerance, memory and direction following while participating in a table top activity. Pt required min. cues for concentration. Pt demonstrated willingness to participate in activity initiated conversation. Pt tolerated session well. Cont. POC.” This Pt was a stroke patient with symptoms of overall weakness.
To begin with the types of writings I will do in college, they will include essays, research reports, group presentations, as well as discussion boards. Throughout my college the main goal is for you to be able to produce writing that is clear and organized in every class. Each of these is a different audience; communicating to all parties in writing is difficult, as wording must be precise and understandable for all individuals reading the reports. Effective writing skills are utilized heavily in both clinical and public education settings. Reports are intended to be completely objective, focusing only on what the therapist observed and not what they perceived. For example, if a therapist thinks the student is anxious because she stutters, they can only say they observed her twirling her hair while she was speaking with a stutter. Objective writing is key to reports, whether the therapist is writing individualized education programs or insurance letters, and separates prospective speech pathologists in graduate school. Efficient, quality writing cannot be understated as a speech pathologist. Without it, individuals that need services the therapist provides would be at a disadvantage and consequently a lower quality of life. Each nurse also has specific ideas or beliefs about what information should be communicated during a verbal report. Valuable information may not be provided or may be forgotten (Casey & Wallis, 2011). Writing information down may help a nurse recall the data later, but if there is no consistent format for recording the information, it may be lost regardless. Interruptions, distractions, and the frequency of communications also may negatively effect the communication process and contribute to a nurse forgetting to share important information (Cacolice-Hildebrand, 2008). Active listening, which involves reflecting back a meaningful understanding of one’s message, is one of the most important skills in effective communication.
My communication skills and disorganization in planning my school week, need improvement. To work on these specific weaknesses, I have set two goals for myself moving forward with this program. Firstly, I am going to make the goal of focusing on making communication part of my student nursing practice. I am going to do this by implementing one communication skill at the end of each lab practice in term two. Focusing on communicating with my partner in the lab while practicing, making communication part of my care, not just another task to be completed. Being disorganized as a nurse and as a student does not lead to success because it is more productive to be organized. So my second goal is to develop a plan for my upcoming week every Sunday for one hour in term two. Along with my shortcomings, there are fears I have looking into the future, into CPE 1 and term 2. Those are the fears of not being good enough in my practice, and being too young to succeed in the program.
