Significant progress has been made in improving their efficiency and operations.Effectiveness of a health institution - hospitals or nursing homes, depends on its goals and objectives, itsstrategic location, soundness of its operations, and efficiency of its management systems. In order to eliminate the inefficiencies witnessed in many public and private hospitals that serve a number of patients, an integrated approach to handling the daily workload is necessary. There is need for all departments within the hospital to work closely together in ensuring more effective and efficient service deliveries. Each organization must have a structure of the authority defined and distributed to help in the organization of the tasks and functions with the aim of achieving the objectives of the organization. An organization structure offers details of the formal structure of the authority in the organization. It is essential to study the organization structure of the organization in order to understand the functioning of the organization. This paper examines the organization structure of the nursing department in Kingston Centre hospital. The paper will look at the nursing and the nursing service management department in the hospital (Burke, 2013). The hospital is selected because I have been able to interact with the facility many times. I have been able to work in this department hence I understand the organization structure and the services offered in the department.

What separates the organizational structure of a healthcare organization from a business, essentially that the hospital 's organization is chiefly founded on the amalgamation of medical and administrative staff (Carayon, et al., 2014). The organizational structure of the twenty-first century solutions in health care hospitals involves, both divisional and hierarchical structure. In the of the chain of command hierarchy, there are various levels of professional’s that fall under other levels within the facility, and each staff member is organized in regards to departments that are related to their (KSA’s) skills, attributes and job duties (Carayon, et al., 2014). Hospital organization philosophies is based on development of values and ethics, with the understanding on moral principles relating to human conduct. These systems are comprised with the processes in decision making and determining the best actions to consider between the difficult alternatives when pertaining to patient care.
Also, there are many challenges for healing hospitals in five characteristics such as, technology and prescription drugs, business factors, bureaucracy, cynicism, and failed leadership. According to Chapman, it is clear that the importance of loving care has decreased as technology enhancement and scientific learning increased (2007). If we consider business factors, it is true that the way we spent money is what we see as valuable for us. By utilizing this many of the old charitable hospitals became high revenue corporations in these days. Therefore, business factors are crucial. The way we practice healthcare and healthcare organizations are changing due to the pressure to reduce costs, improve the quality of care and to meet rigorous guidelines. This change has forced health care professionals to examine we evaluate our overall performance. Paradise Hospital, Inc. has not had any service improvements since 1995. A physician named Avedis Donabedian (2005) proposed a model for assessing health care quality based on structures, processes, and outcomes. He defined structure as the environment in which health care is provided. This is known as the organizational characteristics such as the measurement of staffing ratios and the number of hospital beds. The process is described as the method by which health care is provided. This represents the communication and interaction seen between doctor and patient. The necessity for the tests and procedures performed. The outcome is defined as the consequence of the health care provided, was there a desirable or undesirable effect.
Healthcare organizations do not contribute to the society’s welfare and impact the society positively therefore the relationship with the stakeholders is poor. With a poor relationship, this causes conflict between the patient and the doctor. Enough financial resources should be allocated to prevent quality of care issues and improve the quality of care to the patients. The moneys allocated can be used to purchase equipments that are beneficial to the care of the patients. There are many issues when it comes to the allocation of resources and should be cost effective to ensure the health benefits of those in the community that are being treated. If the allocations of the resources are not done correctly it can affect the community. “Running a health care organization is a team sport. It is very important that all members of the team-whether on the medical staff, in management or on the board-understand the role of governance and what constitutes effective governance” (Arnwine, 2002). Running a hospital is a difficult task. Several factors need to be seriously thought of and considered in every decision and undertaking. Unfortunately, all the three important factors in governing a hospital is not always in harmony. As likened to a team sport, if the three major components are not working with each other as a team, there will be tension and a great divide will be experienced. And often times, the patients will be in the middle and will be greatly impacted. This writer believes that there are several factors that contribute to the tension that usually exists among the medical staff, the board and administration. One factor is the disconnect, where each entity is not seeing each other eye to eye and their visions may be different from each other. Another factor may be the lack of communication in order to bridge the gap and to build a respectful and a relationship wherein there is trust for each end every member of the group. Often times, the medical staff is concerned with ensuring that patients are cared for in a manner that their practice is protected as well as the patients are getting the appropriate care. Since the hospital is a community based acute care hospital, there are numerous subunits. Under the president of Hospital AB and Health Center, the total organization is divided into seven major units which are controlled by seven senior vice presidents. Each unit under the senior vice president is further subdivided into sub units according to the services being provided. The hospital has a distinct subunit configuration for the nursing departments. Each unit is under either a vice president or a director. The major subunits are perioperative services, nursing practice and education, behavior health services, heart center, out patient women services and perinatal clinic, inpatient women services and pharmacy. Each of these major subunits is further subdivided based on the services being provided. The senior vice president for all these units also oversees the programs for patient safety, patient satisfaction and joint commission accreditation. The departments of imaging services, rehabilitation services, cancer center, breast center and quality management comes under another senior vice president (unknown author, 2005).
Improving the quality of health care system is the main goal of this organization. In this case study we will be talking about the strategic plans being made by the organization for the next decade to deal with the problems of resource management, network growth, patient satisfaction as well as nurse staffing. The readiness of the organization towards catering the citizens' needs for health care will also be discussed in this case study (Goetsch and Davis, 2010).
It takes a combination of theories to make healthcare organizations function at a quality standard. Providing the patients with the best care possible is always the goal for any healthcare organization. It takes, not only the nurses, but all the staff of the organization to achieve this goal. Non-nursing theories exist to help guide all staff and provide an optimal work environment to allow quality care to be given to the patient. In this paper, some non-nursing theories will be examined and the importance of each one will be ranked according to this nurse.
