Reflection on Personal and Community Health

Introduction:
As we move through the semester discussing various health topics, we will observe life perspective
throughs
“two lenses”: one lens
inwards
towards self
, and one lens outwards,
beyond self
.
It is
important to not only recognize how our well-being is influenced by the health of our social and
physical environments, but moreover, how our individual beliefs and actions have a meaningful impact
on societal and planetary well-being.
Instructions:
For this assignment, you will: a) complete a brief lifestyle questionnaire

, and b) reflect on your current
health behaviours; relative health risk; barriers and support to establishing a healthy lifestyle; and
actions for lifestyle change that can positively impact the health of self, community and planetary
systems.
The lifestyle questionnaire evaluates your current lifestyle habits and provides a general assessment
of relative health risk for: nutrition, physical activity, stress management, health & safety and
substance use.
Please note this is a general assessment tool used for informational and educational
purposes only and does not substitute for professional medical advice.
The lifestyle questionnaire is a separate file. It is found in the assignment folder on Moodle.
After
completing the questionnaire and the score interpretation found at the bottom of the questionnaire,
you will write a summary reflection following the questions provided below.
You are to respond concisely and intelligibly using complete sentence, paragraph structure. You may
combine the evaluation for some sections
–
for example, if the scores for eating habits and smoking
sections are both excellent and due to the same factors, you may evaluate them together.
e.g. “
My scores for eating habits and smoking are both excellent. My parents value healthy eating
habits, and discourage fast food intake.
We prepare most meals at home that follow our cultural
customs (specify your culture) and emphasized fibre and vegetable intake (give example of meal
served). Smoking is also strongly discouraged in the household, and no one smokes due to this..."
Your reflections will be entered into Moodle textbox for submission. You are not attaching any files,
and you do not need to provide a copy of your completed questionnaire. So as not to lose any work
due to computer glitches, it is recommended that you first write your reflection in a word processing
application, and then copy/paste it into the Moodle assignment textbox. Be sure to click "Submit" at
the bottom of the Moodle page when you have finished. Your reflection will be graded according to the
rubric on the last page of this instruction sheet.

For this question, you are reflecting on your lifestyle behaviours, as assessed in the Lifestyle
Questionnaire.
For your responses, be sure to state the rating (Excellent, Good, At-Risk, Seriously
At-Risk) for all 6 of the lifestyle behaviours. You do not need to state your numerical rating (e.g.
5/10, 7/10); rather state the category (e.g. Good, At-Risk). Additionally, you do not provide a copy
of the completed questionnaire.

“Good” or
"Excellent" range for a category of health: Reflect on how
community* and family support, environment, and personal beliefs/self-education have influenced
your health habit(s) in a positive way, highlighting a personal challenge you overcame. In what
areas would you still like to make improvements, and what change(s) would you make? What
supports (education, community/social, services, environment, finances, etc) might guide and
support to maintain/improve your lifestyle habits?

Risk” for a category of health
: Reflect on how
community* and family support, environment, and personal beliefs/self-education have influenced
your health habit(s), and any challenges you have or continue to experience. What small but
specific lifestyle change(s) could you make to improve your health? What supports (education,
community/social, services, environment, finances, etc) might guide and support you through
positive change in lifestyle habits?

you have experienced either support for OR systemic barrier to achieving optimal health and
well-being.

In reflecting on your global village, how could you engage (now or in the future) in supporting
the health and wellness of others and/or planetary systems? What would you do and who
