There were a couple of reforms that I felt were important towards medical education in the mid-nineteenth century that Ludmeer outlines. First, the American Medical Association (AMA) had a large role in advocating for reforms in medical education. This organization aimed to improve the standards and quality of medical education in the United States. For example, “Throughout the establishment of entrance requirements, longer terms of instruction, and more rigorous requirements for graduation, the AMA hoped that the empirics and sectarians could be squelched and that the prestige of the profession could be reestablished,” (Ludmeer 27). The AMA expressed the importance of a scientific approach to medicine. It was something that encouraged medical schools to try and integrate scientific principles and research methods into their curriculum, in order to have a more evidence-based and rigorous education. The AMA played a crucial role because medical education was highly variable in terms of curriculum, admission requirements, and the quality of training before the AMA’s involvement. Without a reform effort like this, the lack of standardization may have persisted, which would result in inconsistency and large disparities among medical schools. Another reform that I think has a large role in medical education in the early nineteenth century is the Flexner Report written by Abraham Flexner. Similarly to the AMA, the Flexner Report showed the need for standardized medical education. The report called for uniform admission requirements, a strong curriculum, as
well as accreditation standards for medical schools. Flexner personally vidited and evaluated the existing medical schools in the United States and Canada. This meant that his recommendations were influential in determining which schools were meeting the proposed standards and which were not. As a result of the Flexner Report, many substandard medical schools were closed or forced to improve. This led to a significant reduction in the number of medical schools in the United States, but also ensured a higher quality of education. Flexner was important because he advocated for medical schools to be closely affiliated with hospitals to provide students with hands-on clinical experience. I think there is an ongoing need for reforms today in medical training. The field of medicine is constantly evolving with new discoveries, technologies, and treatment modalities. Medical training programs need to be prepared for these advancements to ensure that healthcare professionals are well-prepared to apply the latest knowledge and techniques in their field. Diversity and overall inclusion is also largely important. Ensuring diversity and inclusion in medical education is crucial when trying to address healthcare disparities and improving the overall representation of underrepresented groups in the medical profession. Reforms should focus on creating inclusive learning environments that reflect the diversity of the patients we have.
